摘要
目的:探讨中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)和系统免疫炎症指数(SII)对慢性阻塞性肺疾病急性加重期(AECOPD)并发肺动脉高压(PH)的预测价值。方法:选择2018年1月-2020年3月笔者所在医院呼吸内科接诊的88例AECOPD患者,按照是否并发PH将其分为PH组(46例)和肺动脉压正常组(42例)。采集两组5 ml空腹肘静脉血,测定NLR、PLR、SII。比较两组NLR、PLR、SII值。比较不同PH严重程度患者NLR、PLR、SII值,分析肺动脉收缩压(SPAP)与NLR、PLR、SII值的相关性。结果:PH组NLR、PLR、SII值分别为(5.17±1.24)、(220.15±11.47)、(1 454.24±41.84),均高于肺动脉压正常组的(2.41±0.72)、(124.42±6.54)、(319.57±24.61),差异有统计学意义(P<0.05)。中重度PH组NLR、PLR、SII值均高于轻度PH组,差异有统计学意义(P<0.05)。NLR、PLR、SII值与SPAP呈正相关(P<0.05)。结论:NLR、PLR、SII值与AECOPD并发PH及其严重程度密切相关,可作为评估患者病情严重程度的常用指标。
Objective:To investigate the predictive value of neutrophil to lymphocyte ratio (NLR),platelet to lymphocyte ratio (PLR) and system immune inflammation index (SII) in the acute exacerbation of chronic obstructive pulmonary disease (AECOPD) complicated with pulmonary hypertension (PH).Method:Eighty-eight patients with AECOPD admitted in the department of respiratory medicine of our hospital from January 2018 to March 2020 were selected and divided into the PH group (46 cases) and the pulmonary artery pressure normal group (42 cases) according to whether they were complicated with PH.A total of 5 ml fasting cubital venous blood was collected from the two groups to determine NLR,PLR,and SII.The NLR,PLR and SII values of the two groups were compared.The NLR,PLR and SII values of patients with different PH severity were compared.The correlation between pulmonary artery systolic pressure (SPAP) and NLR,PLR and SII was analyzed.Result:The values of NLR,PLR and SII in the PH group were (5.17±1.24),(220.15±11.47),(1 454.24±41.84),respectively,which were higher than (2.41±0.72),(124.42±6.54),(319.57±24.61) in the pulmonary artery pressure normal group,the differences were statistically significant (P<0.05).The values of NLR,PLR,and SII in the moderate and severe PH group were all higher than those in the mild PH group,and the differences were statistically significant (P<0.05).The values of NLR,PLR,and SII were positively correlated with SPAP (P<0.05).Conclusion:The values of NLR,PLR,and SII are closely related to the AECOPD complicated with PH and its severity,and can be used as common indicators to evaluate the severity of patient’s condition.
