急性脑梗死患者血栓弹力图与凝血功能相关性分析

Correlation Analysis of Thromboelastogram and Coagulation Function in Patients with Acute Cerebral Infarction

目的:探讨急性脑梗死患者血栓弹力图与凝血功能相关性。方法:对2017年10月-2019年10月在本院治疗的150例急性脑梗死患者的临床资料进行回顾性分析,将其按照疾病严重程度分为轻度组55例,中度组50例,重度组45例;同时以同期的健康体检人群共50名作为健康对照组。检测所有入选者的凝血功能相关指标及血栓弹力图(TEG)参数;分析TEG参数与凝血功能的相关性。结果:重度组的TEG参数中K值、R值均明显低于轻度组和中度组,且α角、MA明显高于轻度组和中度组(P<0.05)。中度组的K值、R值均明显低于轻度组,且α角、MA明显高于轻度组(P<0.05)。轻度组的K值、R值均明显低于对照组,且α角、MA明显高于对照组,差异均有统计学意义(P<0.05)。重度组的DD、FIB值明显高于轻度组和中度组,PT明显低于轻度组和中度组(P<0.05)。中度组的DD、FIB值明显高于轻度组,PT明显低于轻度组(P<0.05)。且轻度组的DD、FIB值明显高于对照组,PT明显低于对照组,差异均有统计学意义(P<0.05)。相关性分析发现,DD、FIB与K值、R值呈负相关性关系,且与α角、MA值、CI呈正相关性关系。血浆凝血酶原时间(PT)、部分凝血活酶时间(APTT)与K值、R值呈正相关性关系,且与α角、MA值、CI呈负相关性关系。结论:血栓弹力图各项参数可反映急性脑梗死患者的凝血功能状况,通过进行两项检查有助于诊断疾病及评估病情。

Objective:To explore the correlation between thromboelastogram and coagulation function in patients with acute cerebral infarction.Method:The clinical data of 150 patients with acute cerebral infarction treated in our hospital from October 2017 to October 2019 were retrospectively analyzed,and they were divided into 55 cases in the mild group,50 cases in the moderate group and 45 cases in the severe group;at the same time,a total of 50 healthy people in the same period were used as the healthy control group.All the coagulation function related indexes and thromboelastogram parameters of all the selected candidates were detected;the correlation between TEG parameters and coagulation function was analyzed.Result:The K value and R value of the TEG parameters in the severe group were significantly lower than those in the mild and moderate groups,and the angle a and MA were significantly higher than those in the mild and moderate groups (P<0.05).The K value and R value of the moderate group were significantly lower than those of the mild group,and the angle a and MA were significantly higher than that of the mild group (P<0.05).The K value and R value of the mild group were significantly lower than the control group,and the angle a and MA were significantly higher than the control group,the differences were statistically significant (P<0.05).The DD and FIB values of the severe group were significantly higher than those of the mild and moderate groups,and the PT was significantly lower than that of the mild and moderate groups (P<0.05).The DD and FIB values of the moderate group were significantly higher than that of the mild group,and the PT was significantly lower than that of the mild group (P<0.05).The DD and FIB values of the mild group were significantly higher than the control group,and the PT was significantly lower than the control group,and the differences were statistically significant (P<0.05).Correlation analysis found that DD and FIB had negative correlations with K and R values,and positive correlations with angles a,MA and CI.Plasma prothrombin time (PT) and partial thromboplastin time (APTT) were positively correlated with K value and R value,and negatively correlated with angle a,MA value and CI.Conclusion:The parameters of thromboelastogram can reflect the coagulation function of patients with acute cerebral infarction.It is helpful to diagnose the disease and evaluate the condition by performing two examinations.
