铝碳酸镁联合兰索拉唑治疗幽门螺杆菌相关性胃溃疡的效果及对复发率的影响

Effect of Hydrotalcite Combined with Lansoprazole in Treatment of Helicobacter Pylori Associated Gastric Ulcer and Its Influence on Recurrence Rate

目的:探究铝碳酸镁联合兰索拉唑治疗幽门螺杆菌(Hp)相关性胃溃疡的效果及对复发率的影响。方法:采用随机数字表法将笔者所在医院2018年1月-2019年7月收治的226例Hp相关性胃溃疡患者分为试验组和对照组,各113例。两组均进行标准四联疗法2周后,对照组继续口服兰索拉唑,试验组在对照组用药方案上增加铝碳酸镁,均进行8周治疗。比较治疗8周后两组疗效及Hp根除率,比较治疗前及治疗8周后两组胃液细胞因子[环氧合酶-2(COX-2)、血管内皮生长因子(VEGF)]水平。比较治疗8周内两组用药不良反应发生情况,比较治疗后6个月时两组Hp相关性胃溃疡复发率。结果:治疗8周后,试验组治疗总有效率及Hp根除率均高于对照组,差异均有统计学意义(P<0.05);治疗8周后,试验组胃液COX-2、VEGF水平均高于本组治疗前及对照组治疗后,差异均有统计学意义(P<0.05);治疗8周内,两组不良反应总发生率比较差异无统计学意义(P>0.05);治疗后6个月时,试验组Hp相关性胃溃疡复发率低于对照组,差异有统计学意义(P<0.05)。结论:铝碳酸镁联合兰索拉唑治疗Hp相关性胃溃疡效果显著,用药方案安全性好,治疗后不易复发。

Objective:To investigate the efficacy of Hydrotalcite combined with Lansoprazole in treatment of Helicobacter pylori (Hp) associated gastric ulcer and its influence on the recurrence rate.Method:A total of 226 Hp associated gastric ulcer patients admitted to the author’s hospital from January 2018 to July 2019 were randomly divided into the experimental group and the control group,113 cases in each group.After 2 weeks of standard quadruple therapy,the control group continued to take Lansoprazole,and the experimental group added Hydrotalcite on the basis of the control group,both for 8 weeks.The efficacy and Hp eradication rates of the two groups were compared after 8 weeks of treatment,and the levels of gastric juice cytokines[cyclooxygenase-2 (COX-2) and vascular endothelial growth factor (VEGF)]were compared before and after 8 weeks of treatment.The occurrence of adverse drug reactions within 8 weeks after treatment was compared between the two groups,and the recurrence rate of Hp associated gastric ulcer 6 months after treatment was compared between the two groups.Result:After 8 weeks of treatment,the total effective rate of treatment and the eradication rate of Hp in the experimental group were both higher than those in the control group,the differences were statistically significant (P<0.05).After 8 weeks of treatment,the levels of gastric juice COX-2 and VEGF in the experimental group were higher than those before treatment and those in the control group after treatment,the differences were statistically significant (P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups within 8 weeks of treatment (P>0.05).At 6 months after treatment,the recurrence rate of Hp associated gastric ulcer in the experimental group was lower than that in the control group,the difference was statistically significant (P<0.05).Conclusion:Hydrotalcite combined with Lansoprazole in treatment of Hp associated gastric ulcer is effective,safe and not easy to relapse after treatment.
