改良进针法针刀松解联合中药离子导入治疗髌下脂肪垫损伤的临床疗效

Modified method of acupotomy combined with Chinese medicine iontophoresis for the treatment of patients with infrapatellar fat pad injury

目的观察改良进针法针刀松解联合中药离子导入治疗髌下脂肪垫损伤的临床疗效。方法收集2016年1月至2018年5月青岛市第六人民医院骨科门诊就诊的髌下脂肪垫损伤患者385例,通过改良进针法针刀松解联合中药离子导入治疗,对治疗前后膝关节疼痛度、膝关节活动范围、总体疗效进行评估。结果本研究纳入的385例髌下脂肪垫损伤患者中,男127例,女258例;年龄26~79岁,平均63.5岁;病史1个月~23年,平均17.4个月。经随访观察,治疗后6个月时患者视觉模拟评分(VAS)(2.4±0.8)分明显低于治疗前(7.3±1.3)分(P<0.05),膝关节活动度(106+9)°明显高于治疗前(85+11)°(P<0.05)。随访6个月时优效96例,良效203例,好转78例,无效8例。结论改良进针法针刀松解联合中药离子导入治疗髌下脂肪垫损伤,可明显提高治疗效果,疗效显著。

Objective To observe the clinical efficacy of modified method of acupotomy combined with Chinese medicine iontophoresis in the treatment of patients with infrapatellar fat pad injury.Methods A total of 385 patients with infrapatellar fat pad injury in the Orthopedic Outpatient Department of the Sixth People's Hospital of Qingdao from January 2016 to May 2018,were collected.The combination of modified acupotomy therapy with traditional Chinese medicine iontophoresis were used for the treament.The pain degree,the range of motion and the overall efficacy of the knee joint were evaluated before and after the treatment.Results In this study,385 patients with infrapatellar fat pad injury were included.Among them,127 were male and 258 were female,the average age was 63.5 years(26-79 years),and the average history was 17.4 months(1 month-23 years).By 6 months follow-up observation,the visual analogue scale(VAS)in all patients was significantly lower(2.4±0.8)than that before the treatment(7.3±1.3)(P<0.05),and the knee joint motion was significantly higher(106+9)°than that before the treatment(85+11)°(P<0.05).The excellent effect was in 96 cases,and the good,poor and ineffective effects were in 203 cases,78 cases,and 8 cases respectively.Conclusion The modified method of acupotomy combined with Chinese medicine iontophoresis has a significant therapeutic effect for the treatment of patients with infrapatellar fat pad injury.
