摘要
目的:掌握临床医师对DRG付费方式改革的认识程度、看法及需求,为下一步全面推进DRG付费提供数据支持。方法:对临床医师开展问卷调查,统计临床医师关于DRG付费方式改革的相关数据,分析数据反映的情况,提出应对办法。结果:涉及全国24个城市、71家医院的114名临床医生完成有效问卷调查,其中,43人(37.72%)未听说过DRG付费方式改革;86人(75.44%)从未参加过DRG培训;最想了解的内容是“DRG付费的细则”,其次是“DRG付费对医生收入的影响”;49人(42.98%)选择“DRG付费有优势,也有劣势”,有32人(28.07%)选择“完全支持,能减少不合理费用”,28人(24.56%)选择“不支持,临床疾病复杂多变,仅靠编码不利于诊疗行为的开展”。结论:临床医师对DRG付费方式改革认知程度远远不足,需要正确引导其提高政策知晓度,并探索适当的激励约束机制鼓励其规范医疗行为,以利于推进DRG改革政策落地。
Objective:To understand the understanding,views and needs of clinicians on the reform of DRG payment mode,so as to provide data support for the further comprehensive promotion of DRG payment.Methods:A questionnaire survey was carried out among clinicians to collect the relevant data about DRG payment reform,analyze the situation refl ected by the data,and put forward the countermeasures.Results:A total of 114 clinicians from 71 hospitals in 24 cities completed a valid questionnaire survey.Among them,43(37.72%)had never heard of DRG payment reform,86(75.44%)had never participated in DRG training,the most wanted to know was DRG payment rules,followed by DRG payment's impact on doctors'income,49(42.98%)chose DRG payment has advantages,and the most wanted to know"DRG payment rules",There are also disadvantages",32(28.07%)choose"full support,can reduce unreasonable costs,28(24.56%)choose no support,clinical diseases are complex and changeable,only relying on coding is not conducive to the development of diagnosis and treatment behavior.Conclusion:Clinicians'cognition of DRG payment reform is far from enough.It is necessary to correctly guide them to improve their awareness of policy,and explore appropriate incentive and restraint mechanisms to encourage them to standardize medical behavior,so as to promote the implementation of DRG reform policy.
