摘要
目的:分析全自动生化分析仪在样本交叉污染情况下检测乙型病毒性肝炎的结果,评价临床是否可共用标本进行免疫生化检测。方法:从2019年6月~2020年6月本院受检血液样本中选择30例拥有不同含量的HBsAg的乙型病毒性肝炎阳性血液标本,同时同样选择30例乙肝病毒检测结果为阴性的血液样本,共同作为本次研究观察对象。依据血液标本采集时间顺序分别在分析仪监测前提下,分析血液标本被轻度污染后酶标仪HBsAg含量及HBV-DNA含量,将检测结果与被污染前数据进行对比,分析吸样污染后对HBsAg及HBV-DNA的结果影响。结果:样本交叉污染前后,生化分析仪提示HBsAg数据结果波动不大,而PCR定量检测时可见HBV-DNA含量有较大波动。结论:临床工作中全自动生化仪器对样本进行检测时可能造成样品被污染情况发生,但是其检测结果只对PCR定量检测HBV-DNA时影响较大,而对酶免HBsAg检测结果并无明显影响,可见,检测仪器自身的状态决定样本可能受到污染的程度,所以在计划应用同一份标本进行生化和免疫检测时,应提前评估生化仪对检测免疫的影响程度,再决定是否采用同一样本进行。
Objective:To analyze the results of automatic biochemical analyzer in the case of cross contamination of samples to detect hepatitis B,and to evaluate whether clinical samples can be shared for immunobiochemical detection.Methods:From June 2019 to June 2020,30 positive blood samples with different content of HBsAg were selected from the blood samples tested in our hospital.Meanwhile,30 blood samples with negative test results of hepatitis B virus were also selected as the objects of this study.According to the time sequence of blood sample collection and under the premise of analyzer monitoring,analyze the HBsAg content and HBV-DNA content of enzyme labeling instrument after blood sample is slightly polluted,compare the test results with the data before pollution,and analyze the influence of sample pollution on HBsAg and HBV-DNA results.Results:Before and after slight pollution,the biochemical analyzer showed that the HBsAg data had little fluctuation,while the PCR quantitative detection showed that the HBV-DNA content had great fluctuation.Conclusion:In the clinical work,the automatic biochemical instrument may cause the sample to be polluted when it tests the sample,but its test result only has a great influence on the quantitative detection of HBV-DNA by PCR,but has no obvious influence on the detection result of enzyme-free HBsAg.It can be seen that the state of the test instrument itself determines the degree of possible contamination of the sample,so it is planned to use the same sample in the plan In biochemical and immunoassay,we should evaluate the influence of biochemical instrument on the test immunity in advance,and then decide whether to use the same sample.
