120急救中运用CRAMS创伤评分对提高患者抢救成功率的效果

Study on the effect of using CRAMS trauma score in 120 emergency treatment on improving the success rate of rescue of patients

目的:探讨CRAMS评分在120急诊救治中的应用价值。方法:选取南通市120急救医疗中心2014-03~2019-03院前急救的164例创伤患者作为研究对象,随机分为实验组与对照组各82例。实验组病例在院前急救现场依照CRAMS评分标准进行评分,对伤情作出精准地判断。对照组病例则基于临床表现和病史的前提下采取传统院前急救模式实施急救诊断。对两组患者的抢救过程中的各项抢救成功率和死亡率进行对比研究。结果:实验组现场明确急救诊断率、现场急救成功率、急诊科延续抢救成功率、转运成功率均比对照组高,差异存在统计学意义(P<0.05);实验组的现场急救中死亡率、转运途中死亡率、急诊科延续抢救死亡率、入ICU或专科病房2h内死亡率均明显低于对照组,差异有统计学意义(P<0.05)。结论:CRAMS评分法简单可行、评估准确,在120急救患者诊治中运用,能够明确急救诊断,显著提高患者的抢救成功率,降低死亡率。

Objective:To explore the application value of CRAMS score in 120 emergency treatment.Methods:A total of 164 trauma patients who received pre-hospital emergency treatment from March 2014 to March 2019 in Nantong 120 Emergency Medical Center were randomly divided into experimental group and control group(82 cases each).Patients in the experimental group were scored in accordance with CRAMS scoring standards on the pre-hospital emergency scene to make accurate judgments on the injuries.Patients in the control group were given the traditional pre-hospital emergency diagnosis mode based on their clinical manifestations and medical history.The success rate and death rate of each rescue in the two groups were compared.Results:The first aid diagnosis rate,first aid success rate,emergency department continued rescue success rate and transport success rate were all higher in the experimental group than in the control group,and the difference was statistically significant(P<0.05);The death rate in the experimental group was significantly lower than that in the control group(P<0.05).Conclusion:CRAMS scoring method is simple,feasible and accurate.It can be used in the diagnosis and treatment of 120 first-aid patients,which can clarify first-aid diagnosis,significantly improve the success rate of rescue of patients and reduce the mortality rate.
