摘要
目的:探究调强放射治疗(Intensity Modulated Radiation Therapy,IMRT)中不同的食管癌肿瘤部位对肺剂量体积参数的影响。方法:基于102例食管癌IMRT治疗病例,将食管癌肿瘤分为中上段,中下段与中段。计算不同组别全肺V5、V10、V20和V30,使用ANOVA检验和非参数Kruskal-Wallis H检验进行分析。结果:对于全肺V5,中上段肿瘤均值(51.67)显著区别于中下段(59.93)与中段(58.03)肿瘤。对于全肺V10、V20和V30,三者之间没有统计学差异(P=0.196,0.311,0.202)。结论:中下段和中段肿瘤的肺剂量体积V5更高,在IMRT计划设计时需要更多关注肺V5的剂量体积限制,以期减少放射性肺炎等肺部损伤的发生。
Objective:To explore the effect of tumor location on pulmonary dose-volume parameters by intensity modulated radiation therapy(IMRT)in patients with esophageal cancer.Methods:All data were divided into three groups according to the tumor location(middleupper group,middle lower group and middle group)based on 102 cases with esophageal cancer.ANOVA test and Kruskal-Wallis H testwere used to analyze V5,V10,V20 and V30 in whole lung.Results:The average values of middle upper groupfor V5(51.67)in whole lung were significantly different from that of middle lower group(59.93)and middle group(58.03).In addition,there were no obvious differences among the three groups for V10,V20 and V30(P=0.196,0.311,0.202).Conclusion:The middle lower group and middle group tend to get higher pulmonary dose-volume(V5),thus more attention should be paid to the dose-volume limit of V5 in IMRT to reduce radiation pneumonitis.
