三维斑点追踪成像技术对慢性阻塞性肺疾病患者左心室收缩功能的评价

3D-STI for the Assessment of Left Ventricular Systolic Function of Chronic Obstructive Pulmonary Disease

目的:应用三维斑点追踪成像技术(3D-STI)评价慢性阻塞性肺疾病(COPD)患者早期左心室收缩功能。方法:选择2016年6月~2018年4月在临床上确诊为COPD患者43例及正常志愿者40例,两组均行常规二维超声心动图和三维斑点追踪成像技术检查,获取左心室舒张末期内径(LVEDd)、左心室收缩末期内径(LVEDs)、左心室舒张末期容积(LVEDV)、左心室射血分数(LVEF)、左心室收缩末期容积(LVESV)以及左心室收缩期整体纵向应变(LVGLS)、径向应变(LVGRS)、圆周应变(LVGCS)及面积应变(LVGAS)。结果:COPD组常规超声测量参数与对照组中LVEDd、LVEDs、LVEDV、LVEF、LVESV相比较,统计学分析差异没有统计学意义(P>0.05);COPD组3D-STI技术测量参数LVGLS、LVGRS、LVGCS、LVGAS均较对照组减低,差异有统计学意义(P<0.05)。结论:3D-STI技术作为一种无创且准确性高的新技术,对COPD患者早期左心室整体收缩功能的临床评价起到了至关重要的作用。

To evaluate the left ventricular systolic function in patients with chronic obstructive pulmonary disease(COPD)by three-dimensional speckle tracking imaging(3D-STI).Methods:From June 2016 to April 2018,43 patients with COPD and 40 normal volunteers were selected,two groups were treated with conventional 2D echocardiography and 3D-STI examination before and after the acquisition of left ventricular end diastolic diameter(LVEDd),and left ventricular end systolic diameter(LVEDs),left ventricular end diastolic volume(LVEDV),left ventricular ejection fraction(LVEF),left ventricular end systolic volume(LVESV),left ventricular global longitudinal strain(LVGLS),left ventricular radial strain(LVGRS),left ventricular circumferential strain(LVGCS)and the left ventricular global area strain(LVGAS).Results:COPD group LVEDd,LVEDs,LVEDV,LVEF,LVESV compared with the control group,there were no significant differences(P>0.05);group COPD LVGLS,LVGRS,LVGCS,LVGAS were lower than those in control group,the difference has statistical significance(P<0.05).Conclusion:3D-STI is a noninvasive and accurate new technology,which plays an important role in the evaluation of left ventricular systolic function in patients with COPD.
