摘要
目的:分析早期气管插管在重型脑出血急救中的应用效果。方法:研究对象为本院2018年2月~2019年7月收治的46例重型脑出血患者,随机分组之后分别给予研究组和对照组患者气管插管治疗和气管切开治疗,观察两组患者治疗前后的格拉斯哥评分和神经功能缺损评分,观察两组患者的手术时间以及院内获得性肺炎发生率。结果:研究组和对照组患者在治疗前的格拉斯哥评分和神经功能缺损评分差异均不显著(P>0.05),治疗后研究组患者的格拉斯哥评分和神经功能缺损评分分别为(11.65±1.63)和(5.35±1.61),均优于对照组患者的(10.19±1.54)和(6.76±1.67),P<0.05。研究组患者的手术时间为(3.52±1.23)min,显著短于对照组患者的(10.84±3.62)min,P<0.05。研究组患者的院内获得性肺炎发生率为17.39%(4/23),显著低于对照组患者的39.13%(9/23),P<0.05。结论:对重型脑出血患者应用早期气管插管可以有效改善患者的神经功能,缩短手术时间,降低院内获得性肺炎发生率,效果显著。
Objective:To analyze the effect of early tracheal intubation in the emergency treatment of severe cerebral hemorrhage.Methods:46 patients with severe cerebral hemorrhage admitted to our hospital from February 2018 to July 2019 were randomly divided into study group and control group.Patients were treated with endotracheal intubation and tracheotomy respectively.Glasgow score and Shenjing function defect score before and after treatment were observed.Operation time and incidence of hospital acquired pneumonia were observed.Results:There was no significant difference in Glasgow score and neurological deficit score between the study group and the control group before treatment(P>0.05).Glasgow score and neurological deficit score of the study group after treatment were(11.65±1.63)and(5.35±1.61)respectively,which were better than those of the control group(10.19±1.54)and(6.76±1.67)(P<0.05).The operation time of the study group was(3.52±1.23)min,which was significantly shorter than that of the control group(10.84±3.62)min(P<0.05).The incidence of hospital acquired pneumonia in the study group was 17.39%(4/23),which was significantly lower than that in the control group 39.13%(9/23),P<0.05.Conclusion:Early endotracheal intubation can effectively improve the neurologic function of patients with severe cerebral hemorrhage,shorten the operation time and reduce the incidence of hospital acquired pneumonia.
