静脉-动脉体外膜肺氧合联合主动脉内球囊反搏治疗急性ST段抬高型心肌梗死患者的护理 被引量:11

摘要 近年来体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)广泛应用于临床。ECMO是将体内静脉血引出体外泵入氧合器,氧合血液排除二氧化碳后回输至体内,代替心肺功能,为抢救治疗赢得宝贵的时间[1]。氧合后血液直接泵入动脉系统的ECMO称为静脉-动脉ECMO(venoarterial ECMO,VA-ECMO),VA-ECMO改善氧合同时能够部分或全部替代心脏功能,减少心脏前负荷、心脏做功,降低心肌氧耗量。主动脉内球囊反搏(intra-aortic balloon pump,IABP)可通过增加冠状动脉血流和降低心脏后负荷来改善心肌的灌注,促进左心室功能恢复。我科于2018年6月4日、7月27日收治2例急性ST段抬高型心肌梗死的患者,治疗中应用了ECMO及IABP,现将护理过程报告如下。
