颈动脉斑块内新生血管与血压及血压变异的相关性分析 被引量:4

Correlation analysis of neovascularization in hypoechoic carotid plaque with blood pressure and blood pressure variability

摘要 目的探讨血压及血压变异与斑块内新生血管(NV)的关系。方法入组颈动脉低回声斑块及行超声造影检查患者,分为斑块内无NV组(N-NV组)和斑块内NV组。连续监测每日血压,计算血压变异系数(BP-CV),规范化表格收集人口特征及临床资料。结果(1)入组患者84例,平均年龄64.3岁,斑块内NV检出率70.2%;(2)NV组与N-NV组性别、SBPmean、DBPmean及DBPcv比较差异有统计意义(P<0.05);(3)Logistic回归分析发现,校正所有变量后,SBPmean、DBPmean及DBPcv与斑块NV风险呈正相关,差异有统计统计学意义(P<0.05),比值比分别为OR=1.104,95%CI(1.036,1.175);OR=1.285,95%CI(1.115,1.481)和OR=1.261,95%CI(1.018,1.561)。结论血压及血压变异与斑块内新生血管均相关,提示不稳定斑块防治应重视血压及血压变异控制。 Objective To investigate the relationship between blood pressure and blood pressure variation and neovascularization(NV)in plaque.Methods Patients with carotid hypoechoic plaques and undergoing contrast-enhanced ultrasound examination were divided into plaque-free NV group(N-NV group)and plaque NV group.Continuously monitor daily blood pressure,calculate the coefficient of variation of blood pressure(BP-CV),and collect demographic characteristics and clinical data in standardized tables.Results Totally 84 patients were enrolled,with an average age of 64.3 years,and the detection rate of NV in the plaque was 70.2%.Comparing the NV group and the N-NV group,the differences in gender,SBPmean,DBPmean and DBPcv were statistically significant(P<0.05).Logistic regression analysis found that after adjusting for all variables,SBPmean,DBPmean and DBPcv were positively correlated with the risk of plaque NV,with statistical differences(P<0.05),and the odds ratios wereOR=1.104,95%CI(1.036,1.175);OR=1.285,95%CI(1.115,1.481)and OR=1.261,95%CI(1.018,1.561).Conclusion Blood pressure and blood pressure variability are related to neovascularization in plaque,suggesting that the prevention and treatment of unstable plaque should pay attention to blood pressure and blood pressure variability.
