炎症性肠病患者参与临床决策的期望与能力对其参与临床决策满意度的影响

Effect of Expectation and Ability of Patients with Inflammatory Bowel Disease Participating in Clinical Decision Making on their Satisfaction with Clinical Participation

目的了解炎症性肠病患者参与临床决策期望、参与能力及参与临床决策满意度的现状,并分析三者的关系,为提高患者临床决策满意度提供参考。方法采用方便抽样法,选取2018年12月—2019年8月在中南大学湘雅二医院消化内科病房、门诊及消化内镜中心就诊的炎症性肠病患者121例作为研究对象。采用一般资料调查表、患者参与临床决策期望量表、患者参与能力量表及患者参与临床决策满意度量表进行调查分析。结果患者参与临床决策期望量表的得分为(52.65±5.73)分,参与能力量表得分为(114.55±15.18)分,参与临床决策满意度量表的得分为(20.45±3.28)分。Pearson相关分析显示,患者参与临床决策期望、参与能力及参与临床决策满意度均两两呈正相关(P<0.05),参与临床决策能力在参与临床决策期望对参与临床决策满意度影响中起部分中介效应。结论炎症性肠病患者参与能力及参与临床决策满意度有进一步提升的空间,医护人员应注重评估患者参与临床决策期望的能力,并采取有效措施提高患者实际参与临床决策的水平及满意度。

Objective To learn the current expectation,ability and satisfaction of patients with inflammatory bowel disease participating in clinical decision making,and to analyze the relationship among the three,so as to provide a reference for improving patients'satisfaction with clinical decision making.Methods A total of 121 patients with inflammatory bowel disease who visited the Gastroenterology Department,Outpatient Department and Digestive Endoscopy Center of the Second Xiangya Hospital of Central South University from December 2018 to August 2019 were selected as the study subjects by convenience sampling.General Information Questionnaire,the Scale of Patient Expectation of Clinical Decision Making Participation,the Scale of Patient Participation Ability and the Scale of Patient Satisfaction with Clinical Decision Making Participation were used for the investigation and analysis.Results The Scale of Patient Expectation of Clinical Decision Making Participation got(52.65±5.73)points,the Scale of Patient Participation Ability got(114.55±15.18)points,and the Scale of Patient Satisfaction with Clinical Decision Making Participation got(20.45±3.28)points.Pearson correlation analysis showed that the expectation,ability and satisfaction of patients participating in clinical decision making were positively correlated pairwise(P<0.05),and the effect of the expectation on the satisfaction was partially mediated by the ability.Conclusion There is still room for further improvement of the ability and satisfaction of patients with inflammatory bowel disease participating in clinical decision making.Medical workers should pay attention to evaluating the ability of patients to participate in clinical decision making,and take effective measures to improve the level and satisfaction of patients who actually participate in clinical decision making.
