无发热新型冠状病毒肺炎患者HRCT影像分析

目的探讨无发热新型冠状病毒肺炎(COVID-19)患者的胸部高分辨率CT(HRCT)影像学特点。方法收集2020年1月24日至2月16日无发热COVID-19患者11例作为观察组,选取同期年龄相仿低热(37.3℃~38℃)和中等发热(38.1℃~39℃)患者14例、15例作为对照组,对比分析三组的影像学表现。结果11例无发热患者中,10例患者影像学异常,病灶累及双肺5例、累及肺叶数27个,均低于低热组及中热组,其中累及双肺及肺叶数与中热组比较差异有统计学意义(P<0.05)。无发热组共84个病灶,以<1cm小斑片及结节且纯磨玻璃影(PGGO)、磨玻璃影(GGO)伴小叶间隔增厚或铺路石征为主,发生率均低于低热组、中热组,但与中热组比较差异有统计学意义(P<0.05);无发热组患者中片状影、以实变为主伴GGO的发生率高于低热组、中热组,且无发热组片状影的发生率与中热组比较差异有统计学意义(P<0.05),以实变为主伴GGO的发生率与低热组、中热组差异有统计学意义(P<0.05)。结论无发热COVID-19患者影像学表现多样,与中、低发热患者具有一定一致性,也存在一些差异。

Objective To investigate the imaging characteristics of high resolution CT(HRCT)in patients without corona virus disease 2019(covid-19).Methods Eleven covid-19 patients without fever in our hospital from January 24 to February 16,2020 were collected as the study group,and 14 and 15 covid-19 patients with similar low fever(37.3℃~38℃)and moderate fever(38.1℃~39℃)in the same period were selected as the control group,and the imaging manifestations of the three groups were compared and analyzed.Results Among the 11 patients without fever,10 had imaging abnormalities,5 had lesions involving both lungs and 27 had lobe number,all of which were lower than those in the low-heat group and the moderate-heat group.There were a total of 84 lesions in the non-febrile group,with small patches and nodules less than lcm,and pure ground glass shadow(PGGO)and ground glass shadow(GGO)accompanied by interlobular septal thickening or paving stone characteristics.The incidence of flaky shadow and consolidation with GGO in the group without fever was higher than that in the low-heat group and the moderate-heat group,and the incidence of flaky shadow in the group without fever was statistically different from that in the moderate-heat group,and the incidence of consolidation with GGO was statistically different from that in the low-heat group and the moderate-heat group.Conclusion Ghe imaging manifestations of COVID-19 without fever are varied,which are consistent with those with moderate and low fever,and there are also some differences.
