血磷管理健康专员干预模式对CKD-MBD病人自我管理行为及钙磷水平的影响

Effect of blood phosphorus management health specialist intervention model on self-management behavior and calcium and phosphorus levels in patients with CKD-MBD

目的:探讨血磷管理健康专员干预模式对慢性肾病矿物质及骨代谢异常(CKD-MBD)病人自我管理行为及钙磷水平的影响。方法:选取2018年1月—2019年1月某院收治的CKD-MBD病人160例为研究对象,根据随机数字表法将病人分为观察组与对照组各80例。对照组行常规健康宣教,观察组成立血磷管理健康专员对病人实施护理干预,比较两组病人干预前后自我管理行为及钙磷水平。结果:干预后观察组病人高磷血症发生原因、高磷血症临床症状、高磷血症危害、高磷血症饮食指导、高磷血症临床治疗等方面知识评分高于对照组(P<0.05)。干预后观察组病人情绪处理能力、问题解决能力、执行自我照顾能力、伙伴关系及自我管理能力评分高于对照组(P<0.05)。干预后观察组病人血磷水平、血钙水平、钙磷乘积较对照组明显下降(P<0.05)。结论:血磷管理健康专员干预模式可提高CKD-MBD病人的自我管理能力,并有助于降低病人的血磷水平。

Objective:To explore the effect of blood phosphorus management health specialist intervention model on self-management behavior and calcium and phosphorus levels in patients with chronic kidney disease-mineral and bone disorders(CKD-MBD).Methods:A total of 160 patients with CKD-MBD admitted to a hospital from January 2018 to January 2019 were selected as the research objects.According to the random number table method,the patients were divided into observation group and control group,80 cases in each.The patients in control group received routine health education,and the patients in observation group received blood phosphorus management health specialist carrying out nursing intervention on the patients.To compare the self-management behavior and calcium and phosphorus levels of the two groups of patients before and after intervention.Results:After intervention,the knowledge scores of the causes of hyperphosphatemia,the clinical symptoms of hyperphosphatemia,the harm of hyperphosphatemia,the dietary guidance of hyperphosphatemia and the clinical treatment of hyperphosphatemia in the observation group were higher than that in the control group(P<0.05).After intervention,the scores of emotional processing ability,problem solving ability,executive self-care ability,partnership ability and self-management ability in the observation group were higher than those in the control group(P<0.05).After intervention,the blood phosphorus level,blood calcium level and product of calcium and phosphorus in the observation group were significantly lower than those in the control group(P<0.05).Conclusions:The blood phosphorus management health specialist intervention model can improve the self-management ability of patients with CKD-MBD and help reduce blood phosphorus levels in patients.
