空心钉内固定联合中医药治疗中年Garden Ⅳ型股骨颈骨折的临床疗效探析

The efficacy of sequential cannulated screw internal fixation plus TCM medicine on middle-aged GardenⅣfemoral neck fracture

目的:探讨临床上空心钉置钉顺序和中医药的联合应用对中年GardenⅣ型股骨颈骨折内固定治疗的影响。方法:对64例新鲜闭合性中年GardenⅣ型股骨颈骨折患者采用闭合正骨手法复位空心钉内固定手术,研究组辅以中医汤药治疗;分析骨折X线片、术后并发症、Harris评价、视觉模拟评分法(Visual Analoguo Scale,VAS)疼痛评分、生活质量评分。结果:3~6个月骨折愈合,研究组、对照组的优良率依次为93.75%(30/32)、81.25%(26/32),研究组的骨折愈合时间、术后并发症发生率、术后Harris评价、VAS疼痛评分、生活质量评分均优于对照组,差异性显著,具有统计学意义。结论:合理的置钉顺序使中年GardenⅣ型股骨颈骨折闭合复位后获得更好的生物力学结构;空心钉内固定联合中医药治疗可以进一步改善并促进骨折良好愈合及术后恢复。

Objective:To investigate the clinical effect of sequential cannulated screw internal fixation plus TCM medicine on middle-aged GardenⅣfemoral neck fracture.Methods:64 patients were treated by sequential cannulated screw internal fixation,and the study group was treated by TCM medicine more.X-ray of fracture,postoperative complications,Harris score,VAS pain score and living ability score were analyzed.Results:Fracture healing time was 3~6 months.The excellent and good rate was 93.75%(30/32)and 81.25%(26/32),respectively.Fracture healing time,postoperative complications,Harris score,VAS pain score,living ability score in the study group were more excellent than the control group,with statistically significant differences(P<0.05).Conclusion:The nailing order in cannulated screw internal fixation treatment was foundations for more biomechanical structure,and combined with TCM can promote fracture healing and postoperative recovery.
