中医辨证治疗心脾两虚型失眠的临床研究

A study on treating insomnia of the Xinpi Liangxu type by TCM syndrome differentiation

目的:探讨中医辨证治疗心脾两虚型失眠的临床效果。方法:将2017年5月-2019年5月本院门诊接诊的60例心脾两虚型失眠患者纳入研究,随机分为两组,各30例。对照组采用西药治疗,观察组采用中医辨证治疗,对比两组临床疗效、中医症状积分、实验室相关指标[包括血清皮质醇(Cortisol,Cor)、去甲肾上腺素(Norepinephrine,NE)、多巴胺(Dopamine,DA)、5-羟色胺(5-Hydroxytryptamine,5-HT)]以及匹兹堡睡眠质量指数(Pittsburgh Sleep Quality Index,PSQI)评分。结果:治疗2周,观察组临床疗效相比于对照组更佳,差异有统计学意义(P<0.05);治疗后,两组体虚、健忘、乏力、腹胀、便溏等中医症状积分低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,观察组Cor、NE、DA水平低于对照组,5-HT水平高于对照组,差异有统计学意义(P<0.05);观察组PSQI评分低于对照组,差异有统计学意义(P<0.05)。结论:采用中医辨证治疗较西药治疗心脾两虚型失眠效果更佳,可有效改善患者临床症状,调节临床相关指标水平,改善患者睡眠质量。

Objective:To investigate clinical efficacy of TCM syndrome differentiation on insomnia of the Xinpi Liangxu type(心脾两虚型).Methods:60 cases were divided into two groups,30 cases in each.The control group was given conventional western medicine,and the observation group was given TCM syndrome differentiation and treatment.The clinical efficacy,TCM syndrome scores,and laboratory related indicators of the two groups were compared.Results:After 2 weeks of treatment,the efficacy in the observation group was better than the control group,with a statistically significant difference(P<0.05).After treatment,the TCM syndrome scores(including physical weakness,forgetfulness,fatigue,abdominal distension,loose stools,etc.)in two groups were lower than before treatment,and those in the observation group were lower than the control group(P<0.05).After treatment,the levels of Cor,NE and DA in the observation group were lower,and 5-HT was higher than the control group(P<0.05).The PSQI score in the observation group was lower(P<0.05).Conclusion:TCM syndrome differentiation and treatment on insomnia of the Xinpi Liangxu type shows good effects,can effectively improve the clinical symptoms,adjust the level of clinical related indicators,and improve the quality of patients’sleep.
