基于公共卫生项目管理模式下高血压病中医健康技术推广示范工程的效果评估

Evaluation of efficacy of the traditional Chinese medicine health technology promotion demonstration project on hypertension based on public health project management mode

目的:通过评价中医健康技术推广示范工程的应用效果,探讨有效的中医推广技术和模式,为今后中医健康技术在高血压病公共管理中提供科学数据。方法:选取两个社区高血压病阴阳两虚证患者,运用整群抽样法,采取抽签的方式决定规范高血压病管理和治疗的对照组及在此基础上进行中医健康技术推广使用的中医健康技术组,观察其干预治疗后的疗效变化。结果:经治疗干预后,两组血压水平均较治疗前下降,中医健康技术组血压水平、降压幅度和达标率与对照组相比都具有统计学意义(P<0.05);降压总有效率比较,中医健康技术组89.11%(90/101)优于对照组75.92%(82/108),两组比较差异具有统计学意义(P<0.05);两组治疗后的中医证候总积分与治疗前比较均具有显著性差异,具有统计学意义(P<0.05),且中医健康技术组优于对照组(P<0.05);中医健康技术组中医证候总有效率96.03%(97/101),对照组总有效率70.37%(76/108),两组间比较有统计学意义(P<0.05);两组患者血浆同型半胱氨酸(Homocysteine,Hcy)水平和臂踝脉搏波传导速度(3Rachial-ankle Pulse Wave Velocity,baPWV)都显著低于同组治疗前,且中医健康技术组低于对照组,两组间比较无论Hcy水平还是baPWV差异均具有统计学意义(P<0.05)。结论:中医健康技术推广对社区高血压病患者的治疗与疾病控制具有积极的促进作用,是提高社区高血压病人群防治效果的有效途径。

Objective:Through the evaluation of traditional Chinese medicine health technology promotion and application effect of demonstration project,explore the effective promotion of technology and mode of traditional Chinese medicine(TCM),for the future of traditional Chinese medicine health technology in high blood pressure in the public administration to provide scientific data.Methods:Patients with hypertension of Yin and Yang(阴阳)deficiency syndrome in two communities were selected and divided into control group and TCM health technology group by cluster sampling method.The control group was treated with standardized hypertension management and treatment,and the TCM health technology group was treated with TCM health technology on the basis of the control group.Observe the change of curative effect after intervention.Results:After treatment,two groups of blood pressure level was decreased,the TCM health technology group blood pressure levels,step-down amplitude and success rate compared with the control group were statistically significant(P<0.05).The total effective rate of lowering blood pressure in TCM health technology was 89.11%(90/101),which was better than 75.92%(82/108)in control group(P<0.05).The total scores of TCM syndromes in the two groups after treatment were significantly different from those before treatment(P<0.05),and the TCM health technology group was better than the control group(P<0.05).Two groups of patients with plasma Hcy levels and baPWV are significantly lower than group before the treatment,and TCM health technology group is lower than the control group,and compared with the two groups regardless of Hcy levels or baPWV,differences were statistically significant(P<0.05).Conclusion:the promotion of TCM health technology is an effective way to improve the prevention and treatment of hypertension in the community.
