胰岛素不同给药方式治疗妊娠期糖尿病对临床疗效及母婴并发症的影响

The clinical effect of different insulin administration methods in the treatment of gestational diabetes mellitus and their influences on maternal and neonatal complications

目的探究胰岛素不同给药方式治疗妊娠期糖尿病(GDM)对临床疗效及母婴并发症的影响。方法选取2017年1月至2018年1月本院收治的60例GDM患者,随机分为分次组和持续组,每组30例。分次组给予胰岛素分次皮下注射,持续组给予胰岛素泵持续皮下注射。比较两组患者血糖达标时间、胰岛素用量;检测治疗前、治疗1个月后两组患者血糖水平;记录产后母婴并发症情况。结果持续组胰岛素用量少于分次组,血糖达标时间短于分次组(P<0.05);治疗1个月后,两组患者血糖水平均低于治疗前,持续组血糖水平低于分次组(P<0.05);持续组母婴并发症发生率低于分次组(P<0.05)。结论胰岛素泵持续注射治疗GDM患者疗效优于分次皮下注射,能减少胰岛素用量,有效控制血糖水平,并减少母婴并发症的发生。

Objective To explore the clinical effect of different insulin administration methods in the treatment of gestational diabetes mellitus(GDM)and their influences on maternal and neonatal complications.Methods 60 GDM patients who were admitted the hospital from January 2017 to January 2018 were enrolled.They were randomly divided into fractional group(n=30)and continuous group(n=30).The fractional group was given fractional subcutaneous injection of insulin,while continuous group was given continuously subcutaneous injection of insulin pump.The compliance time of blood glucose and insulin dosage were compared between the two groups.The blood glucose level in both groups before treatment and after 1 month of treatment were detected.The postpartum maternal and neonatal complications were recorded.Results The insulin dosage in continuous group was fewer than that in fractional group,and compliance time of blood glucose was shorter than that in fractional group(P<0.05).After 1 month of treatment,blood glucose level in both groups was decreased,which was lower in continuous group than fractional group(P<0.05).The incidence of maternal and neonatal complications in continuous group was lower than that in fractional group(P<0.05).Conclusion The curative effect of insulin pump continuous injection is better than that of fractional subcutaneous injection on GDM patients,which can reduce insulin dosage,effectively control blood glucose level,and reduce the occurrence of maternal and neonatal complications.
