自拟补气养血方对局部进展期胃癌化疗相关性骨髓抑制的疗效观察

Observation on the Therapeutic Effect of Nourishing Qi and Blood Prescription on Chemotherapy-Related Myelosuppression in Locally Advanced Gastric Cancer

目的观察自拟补气养血方在局部进展期胃癌中化疗所导致的骨髓抑制的疗效及其对化疗后患者的生活质量的影响,进而探讨补气养血药物在化疗所引起的骨髓抑制中的疗效及作用。方法选取北部战区总医院全军肿瘤诊治中心2017年5—至2019年11月76例进展期胃癌患者作为研究对象,按随机数字表法分为对照组和观察组,每组各38例,对照组给予常规药物化疗,实验组在此基础上加上补气养血方,两组同时治疗3周,观察并对比两组患者化疗前1天,化疗第8天,第15天,第22天,两组血常规、免疫功能指标、生活质量的变化情况,并进行统计学比较。结果①治疗结束后观察组较实验组白细胞(WBC)、中性粒细胞计数、血红蛋白(HGB)、血小板(PLT)均显著升高(P<0.05),补气养血方能有效预防化疗引起的骨髓抑制。②治疗结束后观察组CD4+,CD4+/CD8+,IgE、IgA、IgM水平较治疗前均显著升高(P <0.05),而对照组治疗前、完成治疗后比较,差异则不显著(P> 0.05)补气养血方能提高免疫功能。③化疗前1天,化疗第8天,第15天,第22天观察组疲劳积分低于治疗组(P <0.05),两组差异有统计学意义,补气养血方能有效改善化疗患者的生活质量。结论自拟补气养血方能有效提高进展期局部胃癌患者血常规水平、免疫功能状态,并能有效改善生活质量,从而提高临床疗效,不良反应率低。

Objective To explore the effect ofnourishing Qi and bloodprescriptionon chemotherapy-related myelosuppression in locally advanced gastric cancer. Methods 76 patients with advanced gastric cancer were selected from May 2017 to November 2019,the patients were divided into control group and observation group according to random number table,each group had 38 cases,the Control Group was given routine chemotherapy,the observation group added Chinese medicine of invigorating Qi and blood on this basis,Both groups were treated simultaneously for 3 weeks,Observe the changes of blood routine,immune function and quality of life were observed 1 day before chemotherapy,8 days,15 days and 22 days after chemotherapy. Results①WBC,neutrophil count,HGB and Plt were significantly higher in the observation group than those in the experimental group after treatment (P < 0. 05),the difference was not significant in control group before and after treatment. ②After treatment,the levels of CD4+,CD4+/CD8+,IgE,IgA and IgM in the observation group were significantly higher than those before treatment,the difference was not significant in the control group before and after completion of treatment (P > 0. 05) . ③1 Day before chemotherapy,8 days,15 days and 22 days after chemotherapy,the fatigue score of the observation group was lower than that of the treatment group (P < 0. 05),The difference between the two groups was statistically significant. Conclusion Nourishing Qi and blood prescription can improve the blood routine and immune function of patients with advanced local gastric cancer,thus improving the curative effect and quality of life.
