摘要
数值分析已应用于髋臼周围截骨术的模拟中,以模拟增生性髋关节接触应力集中的改善。由于很难从电子计算机断层扫描(computed tomography,CT)或磁共振成像(magnetic resonance imaging,MRI)图像中分割软骨层,因此假定髋关节模型为简单的完美球窝形关节。通过数值模拟接触分析来阐明球面软骨层建模方法,模拟髋臼周围截骨术中动步态的软骨层接触压力和接触面积,并通过旋转截骨段模拟髋臼周围截骨术。数值模拟结果表明,在球面软骨层模型中观察到相对连续的接触压力分布和较低的峰值接触压力,球面软骨层建模方法能合理地反映软骨层接触压力。髋臼周围截骨术后能明显改善软骨层接触压力分布,从而为髋臼周围截骨术提供生物力学参考数据。
Numerical analysis has been applied to the simulation of periacetabular osteotomy(PAO)to reflect the improvement of stress concentration in dysplastic hip joint.While it is difficult to split cartilage layers from computed tomography(CT)or magnetic resonance imaging(MRI)images,it is assumed that the hip model is a simple perfect spherical joint.To clarify the modeling method of spherical cartilage layer,a numerical simulation and contact analysis were conducted,with which the contact pressure and area of the cartilage layer under the dynamic gait on the acetabular osteotomy could be predicted.In addition,the acetabular osteotomy was simulated by rotating the osteotomy segment.The relatively continuous contact pressure distribution and the lower peak contact pressure could be observed in the spherical cartilage layer model.The spherical cartilage layer modeling method can reasonably reflect the contact pressure of the cartilage layer.The contact pressure distribution of cartilage layer can be obviously improved after the amputation of the hip part,thus providing biomechanical reference data for the periacetabular osteotomy.
