摘要
目的:评估肿瘤异常糖链糖蛋白(TAP)在卵巢癌中的诊断效能。方法:收集2017年1月至2017年12月在苏州大学附属第一医院就诊的女性卵巢癌患者94例及同期体检的健康女性96例;检测全血中TAP、血清中人附睾蛋白4(HE4)、糖链抗原125(CA125)、糖链抗原199(CA199)和癌胚抗原(CEA)水平。分析TAP、HE4、CA125、CA199和CEA与临床病理特征的关系;利用Logistic回归及受试者工作特征曲线(ROC曲线)分析比较TAP与HE4、CA125、CA199和CEA的诊断效能。结果:卵巢癌组血清中的TAP、HE4、CA125和CEA水平明显高于健康对照组(P<0.001);晚期(Ⅲ~Ⅳ期)卵巢癌患者的TAP、HE4和CA125水平明显高于早期卵巢癌患者(Ⅰ~Ⅱ期)(P<0.05);卵巢癌患HE4的水平在不同病理类型卵巢癌患者中比较,差异有统计学意义(P<0.05);ROC曲线分析表明TAP在卵巢癌患者中的诊断效能明显高于其它肿瘤指标(AUC=0.991,95%CI:0.964~0.999,P=0.001)。结论:全血中TAP水平的增高可能与卵巢癌相关,TAP联合HE4可以作为卵巢癌筛查的潜在标志物。
Objective:To evaluate the clinical efficacy of tumor abnormal protein(TAP)in the diagnosis of ovarian cancer.Methods:94 female patients with ovarian cancer admitted to the First Affiliated Hospital of Soochow University from January 2017 to December 2017 and 96 healthy women who received physical examination during the same period were collected.The levels of TAP in whole blood,human epididymal protein 4(HE4),sugar chain antigen 125(CA125),sugar chain antigen 199(CA199),and carcinoembryonic antigen(CEA)in serum were detected.Analyze the relationship between TAP,HE4,CA125,CA199,and CEA and clinicopathological characteristics.The logistic regression and ROC curve analysis were were used to analyze and compare the diagnostic efficacy of TAP with HE4,CA125,CA199,and CEA.Results:The levels of TAP,HE4,CA125,and CEA in the ovarian cancer group were significantly higher than the healthy control group(P<0.001).The levels of Tap,HE4,and CA125 in patients with late stage ovarian cancer(stageⅢ/Ⅳ)were significantly higher than patients with early stage ovarian cancer(stageⅠ/Ⅱ)(P<0.05).The difference in the comparison of the levels of HE4 in patients with ovarian cancer between patients with different pathological types of ovarian cancer was statistically significant(P<0.05).The ROC curve analysis indicated that the clinical efficacy and predictive value of TAP in the diagnosis of ovarian cancer was greater than the other tumor indicators(AUC=0.991,95%CI:0.964-0.999,P=0.001).Conclusion:The increased level of TAP in peripheral blood is associated with ovarian cancer.It has a great performance for the diagnosis of ovarian cancer.A combination of TAP and HE4 can be possible biomarker in the screening of ovarian cancer.
