自体富血小板血浆联合Ilizarov技术胫骨横向骨搬移术治疗糖尿病足溃疡的临床观察

Autologous Platelet-Rich Plasma Combined with transverse tibial bone transport by Ilizarov technique for treating diabetic foot ulcers

目的观察自体富血小板血浆联合Ilizarov技术胫骨横向骨搬移术治疗糖尿病足溃疡的临床效果。方法选取2017年2月至2019年2月在我院诊治的58例糖尿病足溃疡患者,将其分为对照组(29例)予以Ilizarov技术胫骨横向骨搬移术治疗,实验组(29例)予以自体富血小板血浆联合Ilizarov技术胫骨横向骨搬移术治疗,观察两组临床治疗效果。结果实验组术后1月足部皮温、疼痛评分显著优于对照组,愈合时间显著短于对照组(P<0.05);实验组并发症发生率为34.48%,显著低于对照组的10.34%(P<0.05)。结论自体富血小板血浆联合Ilizarov技术胫骨横向骨搬移术治疗糖尿病足溃疡疗效显著,能有效缩短创面愈合时间,减少并发症,缓解患者疼痛不适感,为糖尿病足的治疗提供新思路。

Objective To observe the clinical effect of autologous platelet-rich plasma combined with transverse tibial bone transport by the Ilizarov technique to treat diabetic foot ulcers.Methods A total of 58 patients with diabetic foot ulcers who were diagnosed and treated in the hospital between February 2017 and February 2019 were selected as the subjects.They were divided into the control group(29 cases)and the experimental group(29 cases).The two groups were treated with transverse tibial bone transport by the Ilizarov technique and autologous platelet-rich plasma combined with transverse tibial bone transport by the Ilizarov technique.The clinical effects in the two groups were observed.Results The foot skin temperature and pain scores of the experimental group at one month after surgery were significantly better than those of the control group,and the healing time was considerably shorter than the control group(P<0.05).The incidence rate of complications in the experimental group(34.48%)was significantly lower than that in the control group'(10.34%)(P<0.05).Conclusion Autologous platelet-rich plasma combined with transverse tibial bone transport by the Ilizarov technique is markedly effective in treating of diabetic foot ulcers.It can effectively shorten the wound healing time,reduce complications,and relieve pain and discomfort of patients,which provides a new idea for the treatment of the diabetic foot.
