穴位埋线治疗肺脾虚寒型中重度变应性鼻炎的临床观察

Clinical observation of catgut implantation at acupoint on the treatment of moderate and severe persistent allergic rhinitis due to deficiency and coldness of lung and spleen

目的观察穴位埋线治疗肺脾虚寒型中重度持续性变应性鼻炎(Allergic Rhinitis,AR)的临床疗效。方法取48例肺脾虚寒型中重度持续性AR患者,分为埋线组与西药组。埋线组采用中医埋线治疗,西药组采用鼻喷丙酸氟替卡松治疗。两组均治疗9周,并于治疗结束及开始治疗18周后进行随访,比较两组临床疗效、生活质量评分。结果疗程结束后埋线组治疗总有效率64.0%,西药组总有效率78.3%,无显著性差异(P=0.278);开始治疗18周后埋线组总有效率76.0%,西药组总有效率39.1%,埋线组总有效率显著高于西药组(P=0.010)。两组疗程结束后鼻结膜炎生存质量调查问卷(Rhinoconjunctivitis Quality of Life Questionnaire,RQLQ)各项分值均较各组治疗前显著降低,但两组间差异均无统计学意义(均P>0.05)。两组开始治疗18周后活动、睡眠、实际问题、鼻部症状及情感5方面评分,埋线组均显著低于西药组(均P<0.05)。结论穴位埋线能有效治疗肺脾虚寒型中重度持续性AR,并能提高其远期生活质量。

Objective To observe the clinical effect of catgut implantation at acupoint on the treatment of moderate and severe persistent allergic rhinitis(AR)due to deficiency and coldness of lung and spleen and its influence on the life quality of the patients.Methods 48 patients with moderate and severe persistent allergic rhinitis(AR)due to deficiency and coldness of lung and spleen were divided into two groups:Catgut implantation group and Western medicine group.The Catgut implantation group was treated with traditional Chinese medicine and the Western medicine group with fluticasone propionate.The two groups were treated for 9 weeks,and were followed up at the end of treatment and 18 weeks after the start of treatment.Results At the end of treatment,the total effective rate was 64.0%in the Catgut implantation group and 78.3%in the Western medicine group,with no significant difference(P=0.278);After 18 weeks of treatment start,the total effective rate was 76.0%in the Catgut implantation group and 39.1%in the Western medicine group,with the total effective rate significantly higher than that in the Western medicine group(P=0.010).The scores of Rhinoconjunctivitis Quality of Life Questionnaire(RQLQ)in the two groups were significantly lower than those before treatment,but there was no significant difference between the two groups(All P>0.05).After 18 weeks of treatment start,the scores of activity,sleep,actual problems,nasal symptoms and emotion in the Catgut implantation group were significantly lower than those in the western medicine group(All P<0.05).Conclusion Catgut implantation at acupoint can effectively treat AR of moderate and severe persistent allergic rhinitis due to deficiency and coldness of lung and spleen,and improve its long-term quality of life.
