喉癌患者术后放疗所致急性放射性咽喉炎的影响因素分析

Study On Influencing Factors Of Acute Radiation-induced Pharyngitis Among Laryngeal Cancer Patients Who Managed Postoperative Radiation

目的探讨喉癌术后放疗所致急性放射性咽喉炎的影响因素。方法本文收集了2016年至2019年期间53例喉癌术后放疗患者的临床资料,回顾性分析急性放射性咽喉炎的发生率及严重程度,并对患者的年龄、性别、分型、分期、手术方式、放疗方案、化疗情况、吸烟饮酒史以及合并症等临床特征进行单因素分析及多因素logistic回归分析。结果在53例喉癌术后放疗患者中,有25例(47.2%)出现重度(II-IV级)急性放射性咽喉,28例(52.8%)患者出现0或I级急性放射性咽喉炎。单因素分析结果显示:不同原发灶手术方式、放疗方案、戒烟情况患者组的重度急性放射性咽喉炎发生率具有统计学差异(P<0.05)。多因素分析结果显示:喉部分切除术、未戒烟是发生重度急性放射性咽喉炎的危险因素(P<0.05)。结论手术方式、戒烟情况是喉癌术后急性放射性咽喉炎的影响因素,较小的原发灶手术范围、持续吸烟的患者重度急性放射性咽喉炎的发生率更高。

Objective To investigate the influencing factors of acute radiation-induced pharyngitis among laryngeal cancer patients who had received postoperative radiation.Methods The clinical data of 53 laryngeal cancer patients who had received postoperative radiation between 2016 and 2019 were retrospective collected.To assess the severity and morbidity of acute radiation-induced pharyngitis and to analyze the influencing factors of acute radiation-induced pharyngitis by using univariate analysis and multivariate logistic regression analysis.The factors analyzed included:age、gender、category、stage、surgical operation method、radiotherapy schedule、chemotherapy history、smoking history、drinking history and comorbidity.Results Among the 53 laryngeal cancer patients,there were 25 patients(47.2%)with severe(level two or worse)acute radiation-induced pharyngitis,and 28 patients(52.8%)with level 0 or level one.Univariate analysis showed that patients had different morbidities of severe acute radiation-induced pharyngitis for the different tumor surgical operation method、the different radiotherapy schedule、current or former smoking,and the differences were statistically significant(P<0.05).Multivariate analysis showed partial laryngectomy and current smoking were the risk factors influencing the severe acute radiation-induced pharyngitis(P<0.05).Conclusions The surgical operation method and smoking cessation are the influencing factors of acute radiation-induced pharyngitis.The morbidity of severe acute radiation-induced pharyngitis increase in patients with smaller scope of operation and current smoking.
