变应性鼻炎控制患者小气道功能状况分析

Analysis of small airway function in patients controlled by allergic rhinitis

目的分析变应性鼻炎(AR)控制患者小气道功能状况,探讨治疗AR对小气道功能保护的临床意义。方法选择确诊的81例中重度AR患者,记录其年龄、性别、肺功能检查及VAS评分。根据是否有通气功能障碍分为2组,其中通气功能正常组51例,通气功能异常组30例,比较伴有小气道功能异常的患者在治疗后的变化。结果入组的81例患者中,有小气道功能异常59例(72.83%),通气功能异常伴小气道病30例;通气功能正常小气道功能障碍29例(49.15%)。2组患者在性别,年龄及VAS评分比较差异无统计学意义(P>0.05)。通气功能正常与通气功能异常患者FEV 1.0/FVC%和MMEF比较差异均有统计学意义(P<0.05)。小气道功能障碍59例患者的VAS评分及反映小气道功能的主要指标MMEF、FEF 0.75及FEF 0.50在治疗4、8及12周末与基线比较差异均有统计学意义(P<0.05)。结论采用INCS规范治疗AR可以有效提高AR患者的小气道功能,同时改善患者症状及疾病对生活质量的影响。

Objective To analyze the small airway function of patients with allergic rhinitis(AR)and explore the clinical significance of treatment of AR on the protection of small airway function.Method 81 patients with moderate and severe AR were selected,The age,gender,lung function test and VAS score were recorded.According to whether they had the ventilatory dysfunction,the patients were divided into two groups,51 cases in the normal ventilatory function group and 30 cases in the abnormal ventilatory function group.The changes in patients with small airway dysfunction after treatment were compared.Results Among the 81 patients,there were 59(72.83%)with small airway dysfunction,30 cases(50.8%)with abnormal ventilation and 29 cases(49.15%)with normal ventilation.There were no significant differences in gender,age and VAS score between the two groups(P>0.05).There were statistically significant differences in FEV1/FVC%and MMEF between patients with normal ventilation and abnormal ventilation(P<0.05).The VAS scores of 59 patients with small airway dysfunction and the main indicators of small airway function MMEF,FEF75 and FEF50 were significantly different from baseline at the 4,8 and 12 weekends of treatment(P<0.05).Conclusion INCS standard treatment of AR can effectively improve the small airway function of AR patients,and improve the patient’s symptoms and the impact of the disease on the quality of life.
