阿立哌唑改善抗精神病药致泌乳素升高及体重增加的临床观察

Clinical Observation of Aripiprazole in Improving Prolactin and Weight Gain Induced by Antipsychotics

目的:探讨阿立哌唑对抗精神病药致泌乳素升高及体重增加的改善效果。方法:选取2018年3月—2020年3月本院收治的84例抗精神病药致泌乳素升高及体重增加的女性患者,按随机数字表法分为对照组和观察组,各42例。两组均维持原有抗精神病药治疗方案,观察组予以口服阿立哌唑5 mg/d。比较两组阳性和阴性症状量表(PANSS)、泌乳素水平和体质量指数(BMI)及不良反应发生情况。结果:两组治疗16周后阳性症状、阴性症状、一般精神病理症状和总分均较治疗前明显下降,且观察组低于对照组,差异有统计学意义(P<0.05);观察组治疗8周、16周后泌乳素水平低于对照组,BMI低于对照组,差异有统计学意义(P<0.05);两组均无严重不良反应。结论:阿立哌唑可降低抗精神病药所致的泌乳素升高及体重增加患者的泌乳素水平,减轻体质量,且增强抗精神病效果,安全可靠。

Objective:To investigate the effect of aripiprazole on improving prolactin and weight gain induced by antipsychotics.Methods:From March 2018 to March 2020,84 female patients with elevated prolactin and weight gain induced by antipsychotics were selected and randomly divided into two groups,42 cases in each group.Both groups maintained the original antipsychotic treatment plan,and the observation group was given aripiprazole 5 mg/d.The positive and negative symptom scale(PANSS),prolactin level,body mass index(BMI)and adverse reactions were compared between the two groups.Results:After 16 weeks of treatment,the positive symptoms,negative symptoms,general psychopathological symptoms and total scores of the two groups were significantly lower than those before treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05);the prolactin level in the observation group after 8 weeks and 16 weeks of treatment was lower than that of the control group,BMI was lower than the control group,the difference was statistically significant(P<0.05);there were no serious adverse reactions in both groups.Conclusion:Aripiprazole can reduce the level of prolactin,reduce body weight and enhance the antipsychotic effect,which is safe and reliable.
