产气肠杆菌耐药性分析及产Ampc酶基因分子流行病学检测

Analysis of drug resistance and molecular epidemiological detection of Ampc gene in Enterobacter aerogenes

摘要 目的统计本院产气肠杆菌临床耐药情况,并检测产Ampc酶基因型。方法选取本院2016年-2018年临床送检的176株产气肠杆菌菌株,纸片扩散法做药敏试验;经典Hodge法筛选出产Ampc酶菌株,PCR扩增其相关酶基因。结果来源于神经内科、重症监护科和神经外科等科室的菌株占大多数;药敏结果提示产气肠杆菌对头孢唑啉、头孢美唑和头孢拉定耐药率分别高达84.66%、75.00%和73.30%;共筛选出41例产Ampc酶菌株,其基因型主要为DHA型、ADC型和ACT-1型。结论本院产气肠杆菌耐药趋势严峻,分离的产Ampc酶菌株基因型主要为DHA型和ADC型。 Objective To investigate the clinical drug resistance of Enterobacter aerogenes in our hospital and to detect the genotype of Ampc-producing Enterobacter.Methods Totally,176 strains of Enterobacter aeruginosa were selected from our hospital,and the drug sensitivity test was performed by disk diffusion method.Ampc—producing strains were screened by classical Hodge method,and their related enzyme genes were amplified by PCR.Results Most of the strains were from neurology,intensive care and neurosurgery departments.Drug susceptibility results showed that the resistance rates of Enterobacter aerogenes to cefazolin,cefmetazole and cefradine were respectively 84.66%/75.00%and 73.30%.41 strains producing Ampc enzyme were screened out,and their genotypes were mainly DHA,ADC and ACT-1.Conclusion The drug resistance trend of Enterobacter aerogenes in our hospital is severe.The genotypes of Ampc-producing strains isolated are mainly DHA and ADC.
