胼胝体角和颞角宽度对特发性正常压性脑积水诊断的临床价值 被引量:2

The clinical value of CA and THW in the diagnosis of idiopathic normal pressure hydrocephalus

摘要 目的:探讨胼胝体角(CA)和颞角宽度(THW)对特发性正常压力脑积水(INPH)的诊断价值。方法:对30例INPH患者、30例(AD)患者和30例正常健康对照组(NC)进行磁共振成像(MRI)检查。结果:INPH组CA(69±10)°明显小于A D组的(109±11)°和N C组的(115±14)°。当使用的临界值为90°,对应INPH敏感性为97%,特异性为88%。INPH组THW为(7.2±2.1)mm明显大于AD组的(4.9±1.9)mm和NC组的(4.2±2.5)mm,使用的临界值为4mm时,敏感性为95%,特异性为82%。结论:测量CA及THW有助于鉴别INPH、AD患者和正常年龄的受试者。 Objective The utility of measuring the corpus Callosal angle(CA)and Temporal horn’s width(THW)for the diagnosis of idiopathic normal pressure hydrocephalus(INPH)was investigated.Methods Magnetic resonance imaging(MRI)was performed in 30 INPH patients,30 Alzheimer’s disease(AD)patients,and 30 normal control(NC)subjects.Measurement of the CA on the coronal MR images of the posterior commissure perpendicular to the anteroposterior commissure plane was performed and THW on the Axial MR images for all subjects.Results The CA of the INPH group was(69±10)°,which was smaller than(109±11)°in the AD group and(115±14)° in the NC group.When using the threshold of 90°,sensitivity of 97%,and specificity of 88% were observed for discrimination of INPH from AD patients.The THW of the INPH group was(7.2±2.1)mm,which was longer than(4.9±1.9)°in the AD group and(4.2±2.5)° in the NC group.When using the threshold of 4.0mm,sensitivity of 95%,and specificity of 82% were observed for discrimination of INPH from AD patients.Conclusions Measuring the CA and THW helps in differentiating INPH patients from AD and normally aged subjects.CA and THW are conducive to the diagnosis of INPH.
