水囊宫腔填塞术联合改良式子宫背带缝扎术在子宫收缩乏力性出血中的应用

Application of water sac compression haemostatic therapy combined with modified uterus strap suture in treating hemorrhage due to uterine contraction fatigue

目的观察剖宫产术中应用水囊宫腔填塞术联合改良式子宫背带(B-Lynch)缝扎术治疗子宫收缩乏力性出血的临床效果。方法回顾性分析剖宫产术中发生子宫收缩乏力性出血的50例产妇的临床资料,根据治疗方案不同分为对照组和观察组,每组25例。对照组采用改良式B-Lynch缝扎术,观察组在改良式B-Lynch缝扎术基础上行水囊宫腔填塞术。比较2组治疗效果及安全性。结果观察组术中出血量、术后24 h出血量、住院时间、止血有效率、术后并发症发生率均优于对照组,差异有统计学意义(P<0.05)。结论水囊宫腔填塞术联合改良式B-Lynch缝扎术用于剖宫产术中子宫收缩乏力性出血的止血效果较好,成功率较高,不仅保留了生育功能,且操作简便、安全。

Objective To observe effect of hemorrhage due to uterine contraction fatigue by using water sac compression haemostatic therapy combined with modified uterus strap(B-Lynch)suture technique in cesarean section surgery.Methods The clinical data of 50 patients with hemorrhage due to uterine contraction fatigue was analyzed retrospectively.According to varied treatment scheme,they were divided into observation group and control group,with 25 cases in each group.Modified B-Lynch suture was used in the control group,while modified B-Lynch suture combined with water sac compression haemostatic therapy was used in the observation group.The treatment effect and safety were compared between two groups.Results The amount of intraoperative bleeding and bleeding volume of 24 h after operation,hospitalization time,hemostatic success rate and postoperative complication rate of observation group were significantly better than those of the control group(P<0.05).Conclusion Modified B-Lynch suture combined with water sac compression haemostatic therapy used in cesarean section has higher hemostatic effect and success rate.It can preserve the fertility function,and is safe and simple in operation.
