不同分娩方式对产妇血清炎性因子及早产儿免疫功能的影响研究 被引量:2

摘要 目的剖宫产与自然阴道分娩对产妇产褥期感染血清炎性因子水平以及早产儿相关免疫指标的影响。方法回顾性分析2017年1月至2019年12月本院分娩产妇190例,按分娩方式分为剖宫产组(98例)和自然阴道分娩组(92例)。观察比较两组产妇产褥感染率、血清白细胞计数(WBC)、C-反应蛋白(CRP)及降钙素原(PCT)水。剖宫产组中早产儿45例,自然阴道分娩组中早产儿42例,观察比较两组早产儿与足月新生儿的脐血中自然杀伤(NK)细胞和免疫球蛋白水平。结果剖宫产组产褥感染率和分娩后血清WBC、CRP、PCT水平明显高于自然阴道分娩组,比较差异有统计学意义(P<0.05);剖宫产组早产儿NK细胞和lgG水平明显低于自然阴道分娩组早产儿,LgE水平高于自然阴道分娩组早产儿,比较差异均有统计学意义(P<0.05)。结论剖宫产分娩方式会提升产妇血清炎性因子水平和产褥感染率的同时,降低早产儿的免疫功能。 Objective To study the influence of Cesarean section and vaginal delivery on puerpera’s infection and inflammatory factors and premature infant’s immune function indicators.Methods 190 puerpera delivered from January 2017 to December 2019 in our hospital were reviewed and assigned to Cesarean section group(98 cases)and vaginal delivery group(92 cases)according to different delivery modes.Puerperal infection rate,white blood cell counts(WBC),C-reactive protein(CRP)and procalcitonin(PCT)levels were observed.45 premature infants were found in Cesarean section group and 42 premature infants in vaginal delivery group.Umbilical natural killer(NK)cells and immune globulin levels were compared.Results Puerperal infection rate,WBC,CRP and PCT levels in Cesarean section group were higher than vaginal delivery group,with statistically significant difference(P<0.05);NK cell and IgG(immunoglobulin G)levels in Cesarean section group were significantly lower than vaginal delivery group while IgE(immunoglobulin E)levels were higher than vaginal delivery group(P<0.05).Conclusion cesarean delivery mode can improve the level of serum inflammatory factors and puerperal infection rate,and reduce the immune function of premature infants.
