事件相关电位P300及Conners父母症状问卷在癫痫共患注意缺陷多动障碍患儿认知及行为中的应用

Application of event-related potential P300 and Conners parent symptom questionnaire in cognition and behavior of children with epilepsy and attention deficit hyperactivity disorder

目的:探讨事件相关电位P300及Conners父母症状问卷对癫痫(EP)共患注意缺陷多动障碍(ADHD)患儿认知及行为的评价作用。方法:选择EP共患ADHD、EP、ADHD及健康儿童各30例,应用P300检查评价各组儿童的认知水平,应用Conners父母症状问卷评价各组儿童的行为情况。结果:与对照组相比,EP+ADHD组、ADHD组及EP组P300潜伏期明显延长、波幅明显降低(P<0.05);与EP组相比,EP+ADHD组P300潜伏期明显延长、波幅明显降低(P<0.05);与ADHD组相比,EP+ADHD组P300波幅明显降低(P<0.05)。与对照组相比,EP+ADHD组及ADHD组品行问题、学习问题、多动-冲动、多动指数四个因子得分均明显增高(P<0.05),EP组品行问题及多动指数两个因子得分均明显增高(P<0.05);与EP组相比,EP+ADHD组学习问题、多动-冲动、多动指数三个因子得分均明显增高(P<0.05)。结论:P300和Conners父母症状问卷是评价癫痫共患ADHD患儿认知和行为损害的有效手段,其操作简单、价格低廉、检测时间短,患儿及家长依从性高。

Objective:To explore the application of event-related potential P300 and Conners parent symptom questionnaire(PSQ)in cognition and behavior of children with epilepsy(EP)and attention deficit hyperactivity disorder(ADHD).Methods:Thirty children with EP and ADHD,30 children with EP,30 children with ADHD and 30 healthy children were enrolled in the study.P300 was used to evaluate the cognitive level of children in each group,and Conners PSQ was used to evaluate the behavior.Results:Compared with the control group,the latency of P300 was significantly longer and the amplitude was significantly lower in EP+ADHD group,ADHD group and EP group(P<0.05);compared with EP group,the latency of P300 was significantly prolonged and the amplitude was significantly decreased in EP+ADHD group(P<0.05);compared with ADHD group,the amplitude of P300 was significantly decreased in EP+ADHD group(P<0.05).Compared with the control group,the scores of conduct problems,learning problems,impulsivity-hyperactivity and hyperactivity index were significantly higher in EP+ADHD group and ADHD group(P<0.05),and the scores of conduct problems and hyperactivity index were significantly higher in EP group(P<0.05);compared with EP group,the scores of learning problems,impulsivityhyperactivity and hyperactivity index were significantly higher in EP+ADHD group(P<0.05).Conclusion:P300 and Conners PSQ are effective means to evaluate the cognitive and behavioral impairment of children with EP and ADHD.Their simple operation,low price,short detection time and high compliance of children and parents are worthy of clinical promotion and application.
