摘要
目的探讨宫颈癌组织的PIK3CD反义RNA 1(PIK3CD-AS1)水平和临床意义。方法采用实时定量PCR(qPCR)检测本院2015年1月至2018年12月手术切除的89对宫颈癌组织和癌旁组织的PIK3CD-AS1水平,分析不同组织和宫颈癌不同临床病理特征的PIK3CD-AS1水平差异,受试者工作特征(ROC)曲线评估PIK3CD-AS1水平鉴别宫颈癌的临床效能;Kaplan-Meier Plotter在线分析PIK3CD-AS1水平与宫颈癌预后的关系。结果宫颈癌组织的PIK3CD-AS1水平为0.504±0.268,低于癌旁组织的1.587±0.735(t=13.054,P<0.001);ROC曲线分析显示组织PIK3CD-AS1水平诊断宫颈癌的曲线下面积为0.907(95%CI:0.859~0.954),取0.979为截断值时约登指数最大,灵敏度和特异度分别为98.88%和78.65%。以组织PIK3CD-AS1水平的中位值(0.509)为参考分为低水平(≤0.509)组45例和高水平(>0.509)组44例,PIK3CD-AS1水平与FIGO分期、淋巴结转移和肿瘤浸润深度有关(P<0.05),而与年龄、肿瘤大小、病理类型和分化程度无关(P>0.05)。Kaplan-Meier Plotter在线分析结果显示,304例宫颈癌患者PIK3CD-AS1高水平者90例的中位总生存期为95.30个月,长于PIK3CD-AS1低水平者214例的25.77个月,差异有统计学意义(HR=0.43,95%CI:0.24~0.79,P=0.005)。结论宫颈癌组织的PIK3CD-AS1水平降低,且与分期晚、淋巴结转移和深度浸润有关,对辅助宫颈癌诊断和预后预测有一定价值。
Objective To investigate the level and clinical significance of PIK3CD antisense RNA 1(PIK3CD-AS1)in cervical cancer tissues.Methods Real-time quantitative PCR(qPCR)was used to detect the PIK3CD-AS1 levels in 89 pairs of cervical cancer tissues and adjacent tissues surgical resected in our hospital from January 2015 to December 2018.The difference of PIK3CD-AS1 levels in different tissues and different clinicopathological characteristics of cervical cancer were analyzed.The clinical efficacy of PIK3CD-AS1 level in differential diagnosis of cervical cancer was evaluated by receiver operating characteristic(ROC)curve.Relationship between PIK3CD-AS1 level and prognosis of cervical cancer was analyzed online by Kaplan-Meier Plotter.Results PIK3CD-AS1 level in cervical cancer tissues was 0.504±0.268,lower than 1.587±0.735 in adjacent tissues(t=13.054,P<0.001).ROC curve analysis showed that the area under the curve of PIK3CD-AS1 was 0.907(95%CI:0.859-0.954).When 0.979 was taken as the cut-off value,the Youden index was the highest with sensitivity of 98.88%and specificity of 78.65%.According to the median value of tissue PIK3CD-AS1 level(0.509),89 tissues were divided into 45 cases of low-level(≤0.509)group and 44 cases of high-level(>0.509)group.PIK3CD-AS1 level was related to FIGO stage,lymph node metastasis and tumor invasion depth(P<0.05),but not with age,tumor size,pathological type and differentiation degree(P>0.05).Kaplan-Meier Plotter online analysis showed that the median overall survival of 90 patients with high-level of PIK3CD-AS1 among 304 patients was 95.30 months,which was longer than 25.77 months of 214 patients with low-level of PIK3CD-AS1(HR=0.43,95%CI:0.24-0.79,P=0.005).Conclusion The level of PIK3CD-AS1 in cervical cancer tissue was decreased,which was related to late stage,lymph node metastasis and deep invasion.It has certain value in assisting the diagnosis and prognosis prediction of cervical cancer.
