摘要 目的利用Meta分析方法系统评价冷冻切片在乳腺病变术中的诊断价值。方法通过计算机检索PubMed、Medline、Web of science、Cochrane Library(2018年第2期)、中国知网数据库、万方、维普、中国生物医学文献数据库。检索时间为建库至2020年5月1日。筛选文献、提取资料并评价文献质量。采用Revman 5.3、stata 14和Meta-disc软件进行统计学分析。结通过计算机检索PubMed、Medline、Web of science、Cochrane Library(2018年第2期)、中国知网数据库、万方、维普、中国生物医学文献数据库。检索时间为建库至2020年5月1日。筛选文献、提取资料并评价文献质量。采用Revman 5.3、stata 14和Meta-disc软件进行统计学分析。结果最终纳入文献10篇,样本量26284例。Meta分析结果显示,冷冻切片在乳腺病变术中的误诊率为3%,漏诊率为4%。SEN拟合=0.97[95%CI(0.96,0.97)],SPE拟合=0.96[95%CI(0.96,0.96)];+LR=21.54[95%CI(13.46,34.46)];-LR=0.04[95%CI(0.01,0.12)];DOR合并=599.93[95%CI(489.66,735.03)];AUC=0.9909。结论冷冻切片在诊断乳腺病变中具备较高的价值,在临床实际工作中可积极推广应用。 Objective To systematically evaluate the diagnostic value of cryosections in breast lesions using the method of Meta-analysis.Methods PubMed,Medline,Web of science,Cochrane Library(No.2 of 2018),CNKI database,Wanfang,Weipu,China Biomedical Literature Database were searched by computer.The search time was from the establishment of the database to May 1,2020.Screen literature,extract data and evaluate literature quality.Revman 5.3,stata 14 and Meta-disc software were used for statistical analysis.Results 10 articles were finally included,with a sample size of 26284 cases.Meta-analysis showed that the misdiagnosis rate of frozen sections in breast lesions surgery was 3%,and the missed diagnosis rate was 4%.SEN fitting=0.97 [95%CI(0.96,0.97)],SPEfitting=0.96[95%CI(0.96,0.96)].+LR=21.54 [95%CI(13.46,34.46)],-LR= 0.04 [95%CI(0.01,0.12)].DORcombined=599.93 [95%CI(489.66,735.03)],area under the curve(AUC)=0.9909.Conclusion Frozen sections have high value in the diagnosis of breast lesions and can be actively promoted and applied in clinical practice.
