荆花胃康胶丸治疗初治HP阳性FD

Effect of Jinghua Weikang capsule on the initial treatment of FD with positive HP

目的探讨荆花胃康胶丸辅助治疗初治幽门螺杆菌(HP)阳性功能性消化不良(FD)患者的效果。方法选取2017年6月至2019年8月间我院初治HP阳性FD患者126例,简单随机化法分为观察组(n=63)、对照组(n=63)。对照组予以埃索美拉唑四联疗法治疗,观察组予以荆花胃康胶丸联合埃索美拉唑四联疗法治疗。比较两组疗效、HP清除率、治疗前后症状评分、胃电图参数、胃肠激素[胃泌素(GAS)、胃动素(MTL)]、血清脑-肠轴相关因子[神经肽S受体1(NPSR1)、降钙素基因相关肽(CGRP)和P物质(SP)]。结果观察组治疗总有效率90.48%、HP清除率87.30%高于对照组76.19%、71.43%(P<0.05);治疗后观察组餐后饱胀感、反胃、上腹隐痛、胸骨后疼痛评分低于对照组(P<0.05);治疗后观察组胃电图主频、振幅大于对照组(P<0.05);治疗后观察组血清MTL、GAS水平高于对照组(P<0.05);治疗后观察组血清NPSR1水平高于对照组,CGRP、SP水平低于对照组(P<0.05)。结论荆花胃康胶丸联合埃索美拉唑四联疗法治疗初治HP阳性FD效果显著,能有效清除HP,缓解患者症状,增强胃肠动力,调节胃肠激素及脑-肠轴相关因子水平,促进患者胃肠功能恢复。

Objective To investigate the effect of Jinghua Weikang capsule in the initial treatment of patients with positive helicobacter pylori(HP)complicated with functional dyspepsia(FD).Methods 126 patients with HP-positive FD initially treated in our hospital from June 2017 to August 2019 were selected and divided into observation group(n=63)and control group(n=63)by simple randomization.The control group was treated with esomeprazole quadruple therapy,while the observation group was treated with Jingweikang capsule combined with esomeprazole quadruple therapy.Efficacy,HP clearance rate,symptom scores before and after treatment,electroencephalogram parameters,gastrointestinal hormone[gastrin(GAS),motilin(MTL)],serum brain-gut axis related factors[neuropeptide S receptor 1(NPSR1),Calcitonin gene-related peptide(CGRP)and substance P(SP)]were compared between the two groups.Results The total effective rate and HP clearance rate of the observation group were 90.48%and 87.30%higher than those of the control group(76.19%and 71.43%,P<0.05).After treatment,the scores of fullness,nausea,upper abdominal pain and poststernal pain in the observation group were lower than those in the control group(P<0.05).After treatment,the dominant frequency and amplitude of the electrogastrogram in the observation group were greater than those in the control group(P<0.05).After treatment,serum MTL and GAS levels in the observation group were higher than those in the control group(P<0.05).After treatment,the serum NPSR1 level of the observation group was higher than that of the control group,while the levels of CGRP and SP were lower than those of the control group(P<0.05).Conclusion HP-positive FD treated with Jinghua Weikang capsule combined with esomeprazole quadruple therapy is effective in removing HP,relieving symptoms,enhancing gastrointestinal motivity,regulating levels of gastrointestinal hormones and brain-gut axis related factors,and promoting the recovery of gastrointestinal function in patients.
