自拟中药枳朴解郁汤联合聚乙二醇电解质散(Ⅳ)对慢性功能性便秘的临床疗效观察

Clinical Observation of Zhipu Jieyu Decoction Combined with Polyethylene Glycol Electrolyte Powder(Ⅳ)on Chronic Functional Constipation

目的:观察自拟中药枳朴解郁汤联合复方聚乙二醇电解质散(Ⅳ)对慢性功能性便秘的临床疗效。方法:选择2018年12月至2019年12月就诊于安徽中医药大学第一附属医院肛肠科门诊及住院部的慢性功能性便秘患者,共计60例,随机分成对照组和观察组2组,每组各30例,对照组口服复方聚乙二醇电解质散(Ⅳ),观察组在对照组基础上加服自拟中药枳朴解郁汤(由安徽中医药大学第一附属医院制剂中心统一提供)进行治疗。对照组、治疗组均治疗一个月,疗程结束后观察临床疗效、临床症状积分、大便性状积分、肠道菌群水平及不良反应。结果:疗程结束后,对照组和观察组的临床有效率分别为63.33%和93.33%,两组比较差异具有统计学意义(P<0.05)。治疗后,观察组患者的各临床症状积分(排便困难程度积分、排便时间积分、排便频率积分及生活质量自评量表积分)、大便性状积分及肠道菌群水平(双歧杆菌、乳酸杆菌及大肠杆菌)与对照组相比均明显下降,差异具有显著性(P<0.05);整个治疗过程中所有患者均未出现明显不良反应,监测肝肾功能、血、尿、粪常规及心电图检查均未见异常。结论:自拟中药枳朴解郁汤联合复方聚乙二醇电解质散(Ⅳ)治疗慢性功能性便秘具有良好的临床疗效,有效减少肠道致病菌群水平,从而缓解便秘症状,且安全性好,值得临床上进一步推广使用。

Objective:To observe the clinical efficacy of self-made Chinese medicine Zhipu Jieyu Decoction combined with compound polyethylene glycol electrolyte powder(Ⅳ)on chronic functional constipation.Methods:A total of 60 patients with chronic functional constipation who were admitted to the outpatient and inpatient department of the Anorectal Department of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from December 2018 to December 2019 were selected and randomly divided into two groups:control group and observation group.There were 30 cases in each group,the control group was taken orally with compound polyethylene glycol electrolyte powder(Ⅳ),and the observation group was added with self-prepared Chinese medicine Zhipu Jieyu Decoction(provided by the Preparation Center of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine)on the basis of the control group.treatment.The control group and the treatment group were treated for one month.After the course of treatment,the clinical efficacy,clinical symptom score,stool trait score,intestinal flora level and adverse reactions were observed.Results:After the treatment period,the clinical effective rates of the control group and observation group were 63.33%and 93.33%,respectively,and the difference between the two groups was statistically significant(P<0.05).After treatment,the observation group’s clinical symptom scores(defecation difficulty degree score,defecation time score,defecation frequency score and quality of life self-rating scale score),stool trait score and intestinal flora level(Bifidobacterium,Lactobacillus And Escherichia coli)compared with the control group,the difference was significant(P<0.05);all patients had no obvious adverse reactions during the whole treatment process,monitor liver and kidney function,blood,urine,stool routine and electrocardiogram No abnormalities were seen.Conclusion:The self-made Chinese medicine Zhipu Jieyu Decoction combined with compound polyethylene glycol electrolyte powder(Ⅳ)has a good clinical effect in the treatment of chronic functional constipation,effectively reducing the level of intestinal pathogenic bacteria,thereby alleviating the symptoms of constipation,and has good safety.It is worthy of further clinical application.
