乳房下垂矫正采用改良Lejour术式的临床探讨

目的探讨中重度乳房下垂矫正采用改良Lejour术式的手术效果。方法2013年7月~2020年8月,笔者对来院的21例(42侧)中重度乳房下垂患者,按照Lejour法设计,并适当改良,术中形成乳房下极内外侧乳腺瓣并向上交叉折叠,固定于胸大肌筋膜,重建新乳房下皱襞,观察乳房下垂的改善效果、瘢痕情况、并发症以及患者的满意度。结果本组21例患者术后乳房下垂均得到明显改善,切口瘢痕不明显,未发生乳头乳晕坏死等严重并发症,术后随访3~20个月,平均10.1个月,所有患者对乳房外形满意度高。结论改良Lejour术式改善乳房下垂疗效确切,切口瘢痕不明显、并发症小,远期效果稳定,值得临床采用。

Objective To explore the effect of modified Lejour’s technique in the treatment of Moderate to severe breast prolapse.Methods From July 2013 to August 2020,our hospital accepted 21 patients(42 sides)with Moderate to severe breast prolapse,According to the Lejour’s preoperative design,and modified it appropriately,the inside and outside mammary flaps was formed and fixed on the fascia of pectoralis major muscle,fold under new breast reconstruction,To observe the improvement effect,scar condition,complications and patient's satisfaction after operation.Results In 21 cases,after operation,breast prolapse was significantly improved.the scar of incision was not obvious,There were no serious complications such as nipple and areola necrosis.After 3-20 months followup,the average of which is 10.1 months,all patients were highly satisfied with breast appearance.Conclusions Modified Lejour operation has a definite effect on improving breast prolapse,with simple design,no obvious incision scar,few complication,and stable long-term effect,It is worthy of clinical application.
