CO2点阵激光与局部注射曲安奈德治疗儿童增生性瘢痕疗效对比分析

Comparative analysis of the curative effect of CO2 fractional laser and local injection of triamcinolone acetonide in the treatment of hypertrophic scars in children

目的探究CO2点阵激光与局部注射曲安奈德治疗儿童增生性瘢痕的疗效对比。方法选取自2018年3月~2019年3月间收治的增生性瘢痕患儿72例,依据随机数字表法分成两组,对照组36例局部注射曲安奈德治疗,观察组36例使用CO2点阵激光治疗,比较两组患者临床有效率和并发症,测量治疗前后两组患儿瘢痕长轴长度变化,随访半年观察两组患儿生长发育状况。结果观察组患者治疗总有效率为94.44%,高于对照组的72.22%(P<0.05);观察组不良反应发生率为13.89%,略高于对照组的8.33%(P>0.05),观察组治疗不耐受发生率高于对照组(P<0.05);与治疗前对比,两组患儿瘢痕长轴长度均明显缩短,观察组显著短于对照组(P<0.05);治疗后随访6个月,观察组患儿体重高于对照组(P<0.05),两组患儿头围、胸围差异无统计学意义(P>0.05)。结论相较于局部注射曲安奈德,CO2点阵激光治疗儿童增生性瘢痕效果更显著,对患者的身体发育无明显影响,安全性满足要求,值得推广。

Objective To explore the efficacy comparison between CO2 fractional laser and local injection of triamcinolone acetonide in the treatment of hypertrophic scars in children.Methods A total of 72 children with hypertrophic scars admitted from March 2018 to March 2019 were selected and divided into two groups according to the random number table method.The control group 36 cases were treated with local injection of triamcinolone acetonide,and the observation group 36 cases were treated with CO2 Fractional laser treatment,compare the clinical effectiveness and complications of the two groups of patients,measure the changes in the long axis length of the scars of the two groups before and after the treatment,and observe the growth and development of the two groups of children after half a year.Results The total effective rate of treatment in the observation group was 94.44%,which was higher than 72.22%in the control group(P<0.05);the adverse reaction rate in the observation group was 13.89%,slightly higher than 8.33%in the control group(P>0.05).The incidence of treatment intolerance in the treatment group was higher than that in the control group(P<0.05);compared with before treatment,the long axis length of scars in the two groups was significantly shorter,and the observation group was significantly shorter than the control group(P<0.05);follow-up after treatment At 6 months,the weight of the children in the observation group was higher than that in the control group(P<0.05),and there was no significant difference in head circumference and chest circumference between the two groups(P>0.05).Conclusion Compared with the local injection of triamcinolone acetonide,the CO2 fractional laser has a more significant effect on the treatment of hypertrophic scars in children,and has no obvious effect on the physical development of patients.The safety meets the requirements and is worthy of promotion.
