摘要 目的探究孕期甲状腺功能检测结果及其对胎儿脑发育的影响。方法选择接受产检的300名孕妇纳入研究组,另外抽取300例未接受甲状腺功能检测的孕妇纳入对照组。记录研究组甲状腺功能检测结果,比较两组孕妇妊娠结局及胎儿情况(早产、剖宫产、流产、死胎、新生儿合并疾病);以研究组检测出甲状腺功能异常的孕妇例数为依据,按照1∶1的比例随机抽取甲状腺功能正常的孕妇,比较其胎儿脑发育[贝利婴幼儿发育测量表(BSID)]情况。结果研究组孕妇中TPO-Ab阳性39例(13.00%),甲状腺功能异常31例(10.33%),其中甲状腺功能亢进8例,甲状腺功能减退23例。研究组早产、剖宫产、流产、死胎、新生儿合并疾病发生率均明显低于对照组;正常孕妇BSID中MDI、PDI评分水平均明显高于甲状腺功能异常孕妇(P<0.05)。结论孕期出现甲状腺疾病可能性较大,应当加强孕期甲状腺功能检测,对于改善妊娠结局、胎儿情况,提高胎儿脑发育具有有利作用。 Objective To investigate the detection results of thyroid function during pregnancy and their effects on fetal brain development.Methods 300 pregnant women who received thyroid function detection were collected among pregnant women who underwent physical examination were enrolled in study group,and another 300 pregnant women who did not receive thyroid function detection were collected and included in control group.The detection results of thyroid function were recorded in study group,and the pregnancy outcomes and fetal status(premature delivery,cesarean section,abortion,stillbirth,neonatal complications)were compared between the two groups.Taking the number of pregnant women with thyroid dysfunction detected in study group as basis,pregnant women with normal thyroid function were randomly selected based on 1:1 ratio.The fetal brain development [Bayley Scale of Infant Development(BSID)] was compared.Results Among the pregnant women in study group,there were 39 cases(13.00%)of positive TPO-Ab and 31 cases(10.33%)of thyroid dysfunction,including 8 cases(2.67%)of hyperthyroidism and 23 cases(7.67%)of hypothyroidism.The incidence rates of premature delivery,cesarean section,abortion,stillbirth and neonatal complications in study group were significantly lower than those in control group.The scores of MDI and PDI of BSID scale of normal pregnant women were significantly higher than those of pregnant women with thyroid dysfunction(P<0.05).Conclusion It is more likely that thyroid disease will occur during pregnancy.It is necessary to strengthen the detection of thyroid function during pregnancy so as to have a beneficial effect on improving pregnancy outcomes and fetal status and promote fetal brain development.
