颈部伸屈肌群静力增强训练联合颈椎牵引用于神经根型颈椎病患者的效果 被引量:4

Effect on static force strengthening training of cervical extensor and flexor group combined with cervical traction for patients with cervical spondylotic radiculopathy

摘要 目的探讨颈部伸屈肌群静力增强训练联合颈椎牵引用于神经根型颈椎病(CSR)患者的效果。方法选取2018年4月~2019年4月骨科门诊就诊的CSR患者86例,采用抛硬币法分为观察组和对照组,每组各43例。对照组患者予颈椎牵引治疗,观察组患者在对照组常规颈椎牵引治疗基础上加用颈部伸屈肌群静力增强训练。两组患者均连用8周。观察并比较两组患者治疗前后颈肩疼痛程度及颈椎关节活动度的变化,并比较其临床效果。结果治疗8周后,两组患者的颈肩疼痛VAS评分较治疗前明显下降,颈椎关节活动度较治疗前明显上升(P<0.05或P<0.01),且观察组患者变化幅度较对照组更显著(P<0.05);同时治疗8周后,观察组患者临床总有效率(95.35%)明显高于对照组(81.40%)(χ^2=4.071,P<0.05)。结论颈部伸屈肌群静力增强训练联合颈椎牵引用于CSR患者的效果确切,能明显降低颈肩疼痛,改善颈椎关节活动度。 Objective To investigate the effect of static force strengthening training of cervical extensor and flexor group combined with cervical traction for patients with cervical spondylotic radiculopathy(CSR).Methods A total of 86 patients with CSR who were treated in the outpatient clinic of orthopedics from April 2018 to April 2019 were selected.The patients were divided into observation group and control group by coin tossing method,with 43 patients in each group.Patients in the control group were treated with cervical traction,and patients in the observation group were treated with static force strengthening training of cervical extensor and flexor group on the basis of conventional cervical traction in the control group.Patients in both groups were treated for 8 weeks.The changes of neck and shoulder pain and cervical spine joint motion before and after treatment were observed and compared between the two groups of patients,and their clinical effects were compared.Results After 8 weeks of treatment,the VAS scores of neck and shoulder pain in both groups of patients were significantly lower than those before treatment.Cervical spinal joint activity was increased significantly than before treatment(P<0.05 or P<0.01),and the change in the observation group was more significant than that in the control group(P<0.05);after 8 weeks of concurrent treatment,the total clinical effective rate(95.35%)in the observation group was significantly higher than that in the control group(81.40%)(χ^2=4.071,P<0.05).Conclusion The static force strengthening training of cervical extensor and flexor group combined with cervical traction is effective in CSR patients,which can significantly reduce neck and shoulder pain and improve cervical joint mobility.
