不同手术方式对儿童下睑内翻倒睫矫正的效果比较

Comparison of Di fferent Surgical Methods for Correction of Lower Eyelid Inverted Trichiasis in Children

目的比较不同手术方式对儿童下睑内翻倒睫矫正的效果。方法选取2016年8月至2019年8月本院收治的105例儿童下睑内翻倒睫患者,将患者按照不同的手术方案分组,对照组50例患者采用传统Hotz法治疗,观察组55例采用改良皮肤眼轮匝肌切除联合皮下睑板内固定术治疗,获取两组患者的手术治疗相关数据,并采用统计学方法进行比较分析。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组术后并发症(眼睑外翻、睑球分离、眼睑局部组织淤积、眼睑血肿)发生率、随访1年内疾病复发率低于对照组,差异有统计学意义(P<0.05)。观察组面部美学评分(皮肤瘢痕、皮肤褶皱、下睑睫毛弧度自然、双侧下睑缘对称性)高于对照组,差异有统计学意义(P<0.05)。结论改良皮肤眼轮匝肌切除联合皮下睑板内固定术治疗儿童下睑内翻倒睫的效果显著,并发症发生率与疾病复发率均较低,患者的面部美学满意度更高。

Objective To compare and analyze the eff ect of diff erent surgical methods on children with lower eyelid inverted trichiasis.Methods From August 2016 to August 2019,105 cases of children with lower eyelid inverted trichiasis were treated in our hospital.The patients were divided into diff erent groups according to diff erent surgical schemes,50 cases in the control group were treated with traditional Hotz method,and 55 cases in the observation group were treated with modifi ed skin orbicularis oculi muscle resection,combined with subcutaneous meibomian fi xation analysis.Results The eff ective rate of the observation group was higher than that of the control group,with statistical signifi cance(P<0.05).The incidence of postoperative complications(eyelid ectropion,eyelid ball separation,eyelid local tissue deposition,eyelid hematoma)and disease recurrence rate within 1 year in the observation group were lower than those in the control group,with statistical signifi cance(P<0.05).The facial aesthetic score(skin scar,skin fold,natural lower eyelid eyelash curvature,bilateral lower eyelid margin symmetry)of the observation group was higher than that of the control group(P<0.05).Conclusion The improved skin orbicularis oculi muscle resection combined with subcutaneous meibomian internal fi xation in the treatment of children with lower eyelid inverted trichiasis has signifi cant curative eff ect,lower complication rate and disease recurrence rate,and higher facial aesthetic satisfaction of patients.
