联合检测血清β2-MG、CEA、CA125、NSE及CYFRA21-1对肺癌早期诊断的价值研究 被引量:6

摘要 目的:研究β2-微球蛋白(β2-MG)、CEA、CA125、NSE、CYFRA21-1在肺癌患者中的水平及其在肺癌早期诊断中的价值。方法:随机筛选82例健康志愿者为正常对照组,82例肺癌初治患者作为肺癌组,两组比较β2-MG及CEA、CA125、NSE、CYFRA21-1的水平并计算各指标用于肺癌辅助诊断的灵敏度、特异度;ROC曲线分析各指标诊断价值。结果:肺癌组β2-MG、CEA、CA125、NSE、CYFRA21-1水平均高于健康对照组,差异有统计学意义(P<0.05)。小细胞肺癌患者组中NSE水平较鳞癌组及腺癌组升高,差异有统计学意义(P<0.05),鳞癌患者组CYFRA21-1水平显著高于腺癌患者组,差异有统计学意义(P<0.05),其余各指标不同病理类型组间差异无统计学意义(P>0.05)。与四种肿瘤指标单一诊断比较,五种指标联合检测灵敏度升高,差异有统计学意义(P<0.05),特异度较β2-MG及CEA单一检测升高,差异有统计学意义(P<0.05)。β2-MG、CEA、CA125、NSE、CYRFA21-1诊断肺癌ROC曲线下面积分别为0.947、0.701、0.746、0.608、0.615,五项指标联合诊断肺癌ROC曲线下面积为0.969。结论:肺癌患者血清β2-MG、CEA、CA125、NSE及CYRFA21-1水平均升高;联合检测上述血清指标可提高肺癌患者诊断灵敏度及特异度,有效提高患者诊疗价值。 Objective To study the Level ofβ2-microglobulin(β2-MG),CEA,CA125,NSE,CYFRA21-1 in lung cancer patients and its value in early diagnosis of lung cancer.Method 82 cases were randomly selected as normal reference group,82 cases of lung cancer were treated as lung cancer group,compared two groups of subjects ofβ2-MG,CEA,CA125,NSE,CYFRA21-1 levels and diagnostic sensitivity,specificity.ROC curve analysis indicators Diagnostic value.Results The levels ofβ2-MG,CEA,CA125,NSE and CYFRA21-1 in lung cancer group were higher than those in healthy control group,and the difference was statistically significant(P<0.05).The level of NSE was the highest in the small cell lung cancer patients(P<0.05),and the CYFRA21-1 level in the squamous cell carcinoma group was significantly higher than that in the adenocarcinoma group(P<0.05),there were no significant differences between the different pathological types of other indicators(P>0.05).Compared with the single diagnosis of four tumor markers,the sensitivity of the combined detection of the five indicators was increased,the difference was statistically significant(P<0.05),The specificity was higher than that of single detection ofβ2-MG and CEA,and the difference was statistically significant(P<0.05),the area under the ROC curve ofβ2-MG,CEA,CA125,NSE,CYRFA21-1 was 0.947,0.701,0.746,0.608 and 0.615,respectively.The area under the ROC curve of the 5 indexes was 0.969.Conclusion Bothβ2-MG,CEA,CA125,NSE and CYRFA21-1 levels were elevated in lung cancer patients;β2-MG combined with 4 tumor markers can improve the diagnostic sensitivity and specificity of patients with lung cancer,and effectively improve the value of diagnosis and treatment.
