低频电刺激配合康复锻炼治疗脑卒中患者踝关节功能障碍临床研究 被引量:10

Clinical Study of Rehabilitation Training Combined with Low-Frequency Electrical Stimulation on Post-Stroke Malleolus Dorsiflexion Dysfunction

摘要 目的:通过比较低频穴位电刺激结合康复训练疗法与电针疗法对中风患者踝背屈功能障碍的疗效,选择较优化的疗法,为临床治疗踝背屈功能障碍提供优化的治疗方案。方法:将80例符合纳入标准的中风患者随机分为对照组(n=40)与治疗组(n=40)。对照组给与电针治疗,治疗组接受低频穴位电刺激结合康复训练治疗,持续28 d。治疗前后采用踝关节最大主动背屈角度、步态分析系统和FuglMeyer量表进行疗效判定。结果:治疗28 d后,两组患者的踝关节最大主动背屈角度、Fulg-Meyer评分(下肢)较治疗前均有明显的提高,组内比较差异具有统计学意义(P <0.01),组间比较差异具有统计学意义(P <0.05),治疗组优于对照组;治疗28 d后,两组患者患侧步态参数(步长、步速、步频、患侧双支撑相)较疗前明显改善,组内比较差异具有统计学意义(P <0.01),组间比较差异具有统计学意义(P <0.05),治疗组优于对照组。结论:低频穴位电刺激结合康复训练治疗与电针治疗均可以显著改善中风后踝背屈功能障碍,低频穴位电刺激结合康复训练方法疗效更佳。 Objective: To observe the effect of low-frequency acupoint electrical stimulation combined with rehabilitation training on malleolus dorsalis dysfunction after stroke,thus to optimize the treatment plan and provide a strong basis for improving the clinical efficacy. Methods: 80 patients who met the inclusion criteria were randomly divided into the control group( n = 40) and the treatment group( n = 40). The control group was treated with electro-needling the treatment group was treated with low-frequency acupoint electrical stimulation combined with rehabilitation training,The treatment duration was four weeks. The maximum active malleolus dorsiflexion angle and the Fugl-Meyer scale were observed before and after the treatment to evaluate the efficacy. Results: The maximum active malleolus dorsiflexion angle and the scores of Fulg-Meyer of lower extremity were significantly improved after the treatment compared to those before the treatment in the two groups( P < 0. 01);the improvements in the treatment group were more significant than those in the control group( P < 0. 05). The gait parameters( step length,step speed,step frequency and stand phase of affected side)were significantly improved as well after the treatment in the two groups( P < 0. 01);the improvements in the treatment group were more significant than those in the control group( P < 0. 05). Conclusion: Both low-frequency acupoint electrical stimulation combined with rehabilitation training and electro-acupuncture can improve the functional dysfunction of malleolus dorsiflexion after stroke,but low-frequency acupoint electrical stimulation combined with rehabilitation training has better therapeutic effect.
