老年心力衰竭急性加重住院病人临床特点分析

Clinical Characteristics of Acute Decompensated Heart Failure in Hospitalized Senile Patients

目的探讨老年心力衰竭急性加重病人住院的临床特点,为老年心力衰竭急性加重的防治提供理论依据。方法回顾性分析2017年1月-2017年12月住院急性心力衰竭病人的临床资料,收集65岁及以上病人且以80岁为界,分为高龄组和老年组进行分析。共纳入3914例老年急性心力衰竭住院病人,其中老年组1602例,高龄组2312例。结果两组年龄、男性、住院天数、死亡率、住院费用比较差异均有统计学意义(P<0.05)。老年组前5位住院原因分别是高血压病(18.3%)、慢性阻塞性肺疾病急性发作(13.4%)、冠状动脉粥样硬化性心脏病(12.4%)、脑卒中(7.8%)、其他类型心脏病(6.2%)。高龄组前5位住院原因分别为慢性阻塞性肺疾病急性发作(15.9%)、冠状动脉粥样硬化性心脏病(13.6%)、高血压病(12.2%)、肺炎(7.7%)、脑卒中(6.7%)。结论慢性阻塞性肺疾病急性发作是导致老年特别是高龄病人心力衰竭急性加重入院的重要原因。

Objective To investigate the characteristics of acute decompensated heart failure in hospitalized senile patients,and provide theoretical basis for treatment and prevention of acute decompensated heart failure.Methods Clinical data of hospitalized acute decompensated heart failure patients aged 65~80 years from January 2017 to December 2017 were studied retrospectively.A total of 3914 patients were collected,including 1602 patients in old group and 2312 patients in very old group.Results There were statistically significant differences between the two groups in age,gender,length of stay,mortality and hospitalization expenses(P<0.05).The top 5 factors for acute decompensated heart failure in old group were hypertension(18.3%),acute exacerbation of chronic obstructive pulmonary disease(13.4%),coronary artery disease(12.4%),stroke(7.8%),other type heart disease(6.2%).The top 5 factors in very old group were acute exacerbation of chronic obstructive pulmonary disease(15.9%),coronary artery disease(13.6%),hypertension(12.2%),pneumonia(7.7%),stroke(6.7%).Conclusion Acute exacerbation of chronic obstructive pulmonary disease is the most important reason of admission for the old patients,especially for the very old patients with acute decompensated heart failure.
