对比研究sST2与NT-proBNP诊断心衰合并肾功能不全的价值

Comparative study of sST2 and NT-proBNP in the diagnosis of heart failure with renal insufficiency

目的分析sST2与NT-proBNP诊断心衰合并肾功能不全的价值。方法收集医院2017年1月~2019年1月80例心衰合并肾功能不全患者纳入实验组,另再收集20例肾功能正常的心衰患者作为对照组,对两组患者血清sST2与NT-proBNP指标观察,同时对比sST2与NT-proBNP诊断心衰合并肾功能不全的敏感性与特异性。结果两组患者血清指标水平对比,实验组患者sST2、NT-proBNP水平与对照组比较,差异均有统计学意义。sST2与LVEF、NYHA分别保持负相关、正相关,NT-proBNP与eGFR、LVEF保持负相关,同NYHA分级保持正相关,sST2水平与NT-proBNP保持正相关。通过两种指标对患者心功能分级ROC曲线下AOC观察,对比二者在评估患者心功能分级AOC方面的特异度与灵敏度,可发现sST2检测下特异性与灵敏度更高。结论sST2比NT-proBNP在诊断心衰伴肾功能不全中具有更高的敏感性和特异性,对心力衰竭患者能做出更准确和完整的评估,影响对患者预后的判断价值,指导临床医学实践。

Objective To analyze the value of sST2 and NT-proBNP in the diagnosis of heart failure complicated with renal insufficiency.Methods Eighty patients with heart failure and renal insufficiency from January 2017 to January 1919 were enrolled in the experimental group.Another 20 patients with normal renal failure were enrolled as control group.Serum sST2 and NT were measuretl in the two groups.The-proBNP index was observed and the sensitivity and specificity of sST2 and NT-proBNP in the diagnosis of heart failure with renal insuffiriency were rompared.Results The serum levels of the two groups were compared.The sST2 and NT-proBNP levels in the experimental group were significantly different from those in the control group.sST2 was negatively correlated and positively correlated with LVEF and NYHA.NT-proBNP was negatively rorrelated with eGFR and LVEF,and positively correlated with NYHA classification.sST2 level was positively rorrelated with NT-proBNP.The AOC observation of the heart function grading HOC curve by two indicators,comparing the specificity and sensitivity of the two in evaluating the cardiac funrtion grading AOC,can be found that the sperifirity and sensitivity of sST2 deleclion is higher.Conclusion sST2 has higher sensitivity and specificity in the diagnosis of heart failure with renal insufficiency than NT-proBNP.It can make a more arrurate and complete assessment of heart failure patients,affect the judgment value of patients'prognosis,and guide clinical prartire.
