他莫昔芬在多囊卵巢综合征不孕中的应用价值

Application value of tamoxifen in infertility with polycystic ovarian syndrome

目的分析多囊卵巢综合征(PCOS)不孕患者在治疗中应用他莫昔芬的价值。方法选取2017年7月-2018年12月东莞市第三人民医院收治的200例PCOS不孕患者。按照随机数字表法将其分为研究组和常规组各100例。常规组给予氯米芬治疗,研究组给予他莫昔芬治疗。比较两组患者治疗前后卵巢体积、卵泡数量、排卵率、受孕率、促黄体生成素(LH)、卵泡刺激素(FSH)、睾酮(T)等指标水平。结果治疗后,两组患者的卵巢体积与卵泡数量明显低于治疗前,且研究组明显低于常规组,差异均有统计学意义(P<0.05)。研究组患者的排卵率和受孕率明显高于常规组,差异均有统计学意义(P<0.05)。治疗后,两组患者的LH和T水平均明显低于治疗前,而FSH水平则明显高于治疗前,且研究组LH、FSH以及T水平均明显优于常规组,差异均有统计学意义(P<0.05)。结论他莫昔芬在PCOS不孕中的应用,可以控制卵巢体积和卵泡数量,提高排卵率和受孕率,有效调控患者临床激素指标水平,具有较高的临床应用价值。

Objective To analyze the application value of tamoxifen in the treatment of infertility patients with polycystic ovarian syndrome(PCOS). Methods From July 2017 to December 2018,200 infertile patients with PCOS admitted to Dongguan Third People’s Hospital were selected. According to the random number table method,they were divided into study group and conventional group with 100 cases in each group. The conventional group was treated with clomiphene,and the study group was treated with tamoxifen. The ovarian volume,number of follicles,ovulation rate,pregnancy rate,luteinizing hormone(LH),follicle stimulating hormone(FSH),testosterone(T)and other indicators were compared between the two groups before and after treatment. Results After treatment,the ovarian volume and number of follicles in the two groups were significantly lower than before treatment,and the study group was significantly lower than the conventional group,and the differences were statistically significant(P<0.05).The ovulation rate and pregnancy rate of the study group were significantly higher than those of the conventional group,and the differences were statistically significant(P<0.05). After treatment,the levels of LH and T in the two groups were significantly lower than those before treatment,while the level of FSH was significantly higher than that before treatment,and the levels of LH,FSH and T in the study group were significantly better than those in the conventional group,and the differences were statistically significant(P <0.05). Conclusion The application of tamoxifen in patients with PCOS infertility could control the ovarian volume and the number of follicles,improve ovulation rate and pregnancy rate,effectively regulate the level of clinical hormone indicators,which had higher clinical application value.
