摘要
目的探讨不同TOAST分型的脑梗死患者与血清同型半胱氨酸(Hcy)水平的相关性。方法选取2019年1-11月在天津市南开医院治疗的脑梗死患者89例为观察组,选取同期体检中心健康体检者119例为对照组,然后根据TOAST分型对89例患者进行分组,LAA组49例,SAA组37例,CE组3例,检测血清Hcy水平并进行比较。结果观察组的血清Hcy水平明显高于对照组,差异有统计学意义(P<0.05)。观察组根据TOAST分型,LAA比例为55.06%,高于SAA的41.57%,差异有统计学意义(P<0.05)。LAA组和SAA组血清Hcy水平与对照组比较,差异均有统计学意义(P<0.05),但CE组血清Hcy水平与对照组比较,差异无统计学意义(P>0.05)。三个亚组血清Hcy水平两两比较,LAA组、SAA组分别与CE组比较,差异均有统计学意义(P<0.05),但LAA组血清Hcy水平与SAA组比较,差异无统计学意义(P>0.05)。结论血清Hcy水平与脑梗死密切相关,LAA型和SAA型比其他亚型升高更明显,血清Hcy水平对于脑梗死的分型诊断具有一定的临床价值。
Objective To investigate the correlation of serum homocysteine(Hcy)levels in patients with cerebral infarction with different TOAST classification. Methods Eight-nine patients treated in Nankai Hospital of Tianjin from January to November 2019 were selected as the observation group,and 119 healthy persons in the physical examination center of the same period were selected as the control group. Then according to TOAST classification,89 patients were divided into LAA group with 49 cases,SAA group with37 cases and CE group with 3 cases,serum Hcy levels were detected and compared. Results The serum Hcy level of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P <0.05). According to TOAST classification,the proportion of LAA in the observation group was 55.06%,which was higher than 41.57% of SAA,and the difference was statistically significant(P<0.05). There were significant differences in serum Hcy levels between LAA group and SAA group and control group(P<0.05),but there was no significant difference between CE group and control group(P>0.05). The serum Hcy levels of the three subgroups were compared,LAA group was compared with CE group,SAA group was compared with CE group,and the differences were statistically significant(P<0.05). However,there was no significant difference in Hcy level between LAA group and SAA group(P>0.05). Conclusion The level of serum Hcy was closely related to cerebral infarction,LAA and SAA types were more pronounced than other subtypes. Serum Hcy level had certain clinical value for the classification and diagnosis of cerebral infarction.
