电针联合中药灌肠治疗盆腔炎性疾病后遗症慢性盆腔痛临床疗效观察

Clinical Observation of Electroacupuncture Combined with Enema of Traditional Chinese Medicine for Chronic Pelvic Pain with Sequelae of Pelvic Inflammatory Disease

目的观察电针联合中药灌肠对盆腔炎性疾病后遗症慢性盆腔痛患者的影响。方法 56例患者随机分为治疗组和对照组,治疗组予电针联合中药灌肠治疗,对照组予中药灌肠,连续治疗3个月经周期及随访1个月经周期,观察两组临床有效率、下腹部疼痛、腰骶部疼痛VAS评分、局部体征积分、WHO-BREF量表评分的变化。结果 (1)治疗3个月经周期后治疗组总有效率78.57%(22/28),对照组总有效率为57.14%(18/28),两组临床有效率比较差异无统计学意义(P>0.05);(2)治疗3个月经周期治疗组下腹部疼痛VAS评分较治疗前和对照组显著降低(P<0.05);(3)治疗3个月经周期后治疗组患者的子宫压痛、附件区压痛、宫骶韧带触痛评分较治疗前显著降低(P<0.05);(4)治疗3个月经周期后治疗组生存质量感受、健康状况感受、生理领域、心理领域、社会关系领域、环境领域评分均较治疗前及对照组显著增加(P<0.05)。结论电针联合中药灌肠可缓解盆腔炎性疾病后遗症慢性盆腔痛患者的疼痛,改善其生活质量,为盆腔炎性疾病后遗症慢性盆腔痛的治疗提供了可靠的依据。

Objective To observe the clinical effect of electroacupuncture combined with enema of traditional chinese medicine for chronic pelvic pain with sequelae of pelvic inflammatory disease. Mehods 56 patients were randomly divided into treatment group and control group, patients of treatment group were treated by electroacupuncture combined with enema of traditional chinese medicine, patients of control group were treated by enema of traditional chinese medicine.Treatment periods were 3 menstrual cycles and follow-up of 1 menstrual cycle. Clinical efficacy, VAS scores of lower abdominal pain, lumbosacral pain, score of local signs and WHO-BREF were evaluated at baseline and at the end of study. Results(1)After treatment, the clinical effective rate of the treatment group was 78.57%(22/28), and that of the control group was 57.14%(18/28). There was no significant difference between the two groups(P>0.05);(2)After treatment, the VAS score of lower abdominal in the treatment group were significantly lower than that before treatment and those of the control group(P<0.05);(3)The scores of uterine tenderness, adnexal tenderness and uterosacral ligament tenderness in the treatment group after three menstrual cycles were significantly lower than those before treatment(P<0.05);(4)After three menstrual cycles, the scores of quality of life, health status, physiological field, psychological field, social relationship field and environmental field in the treatment group were significantly higher than those before treatment and control group(P<0.05). Conclusion Electroacupuncture combined with traditional Chinese medicine enema can relieve the pain and improve the quality of life, which provides a reliable basis for the treatment of chronic pelvic pain.
