二尖瓣成形术与置换术在老年患者中的效果分析

目的探讨二尖瓣成形术与置换术在老年患者二尖瓣病变治疗中的临床效果。方法回顾性分析210例老年(年龄≥75岁)行二尖瓣手术患者病例资料,对比分析手术方式对术后并发症、再入院率及患者远期生存率的影响。结果二尖瓣成形组与置换组在术后并发症、1年及4年远期生存差异无统计学意义,但老年二尖瓣置换患者术后输注血液制品较多(P<0.05),1年及4年再入院发生率高(P<0.05)。结论老年患者二尖瓣手术是安全可行的,其术后4年生存率约为70%,二尖瓣成形术与置换术术后并发症及远期生存无差别,但二尖瓣置换者术后输血及再入院风险较高。

Objective To explore the clinical effect of mitral valvuloplasty and replacement in the treatment of mitral valve disease in elderly patients.Methods A retrospective analysis of 210 elderly patients(age≥75 years)who underwent mitral valve surgery was conducted to compare and analyze the effects of surgical methods on postoperative complications,readmission rate,and long-term survival rate.Results There was no statistically significant difference in postoperative complications and long-term survival at 1 and 4 years between the mitral valvuloplasty group and the replacement group,but elderly patients with mitral valve replacement had more blood products transfused(P<0.05).The incidence of readmissions was high at 1 and 4 years(P<0.05).Conclusion Mitral valve surgery is safe and feasible in elderly patients.The 4-year survival rate is about 70%.There is no difference in complication and long-term survival after mitral valvuloplasty and replacement surgery,but mitral valve replacement surgery The risk of post-transfusion and readmission is higher.
