下牙槽神经损伤后通过富血小板纤维蛋白进行治疗的临床观察

Effect of PRF on repair of damage in inferior alveolar nerve after removal of impacted teeth

目的探讨富血小板纤维蛋白(platelet rich fibrin,PRF)在阻生齿拔出后引起下牙槽神经损伤的修复过程中的促进作用。方法选取2018年5月—2019年12月因拔牙术后出现下唇感觉异常来本院就诊的患者20例,随机将研究对象分为观察组(11例)和对照组(9例)。观察组给予局麻下清除血凝块,暴露神经创面,PRF置于神经创面,同时也服用甲钴胺治疗,对照组仅给予口服甲钴胺治疗。记录患者治疗前和治疗后1、3、6、12个月下唇感觉异常区域的面积及治愈时间。采用SPSS 16.0软件进行统计学分析。结果观察观察组治愈时间平均为56(四分位数间距)天,短于对照组平均时间78(四分位数间距)天。早期阶段(1~6个月)损伤恢复范围显著高于后期阶段(7~12月),且损伤越轻恢复越快,差异具有统计学意义(P<0.05)。结论富血小板纤维蛋白可以有效促进阻生齿拔出后下牙槽神经损伤修复,缩短治愈时间,减轻病人痛苦。

Objective To investigate the effect of PRF(platelet rich fibrin)on the repair of the damage in the inferior alveolar nerve after the removal of impacted teeth.Methods Twenty patients who came to our hospital from May 2018 to December 2019 due to paresthesia of the lower lip after tooth extraction surgery were selected and randomly divided into observation group(11 cases)and control group(9 cases).The observation group was given local anesthesia to remove blood clots,expose the nerve,PRF was placed on the wound,and also received mecobalamine treatment,while the control group was only given oral mecobalamine treatment.The area and healing time of the paresthesia area of the lower lip were recorded before and after treatment at 1,3,6 and 12 months.SPSS16.0 software was used for statistical analysis.Results The average healing time of the observation group was 56(interquartile interval)days,shorter than that of the control group 78(interquartile interval)days.The range of injury recovery in the early stage(1~6 months)was significantly higher than that in the later stage(July-December),and the lighter the injury,the faster the recovery,the difference was statistically significant(P<0.05).Conclusion Platelet rich fibrin can effectively promote the repair of the damage of the Inferior alveolar nerve after the impacted tooth is pulled out,shorten the healing time and reduce the pain of patients.
